The Brook Hospital is currently seeking an Addiction Services Counselors PRN to join our team . They would assist in covering varied shifts and gain experience working with both adolescent and Adult populations

The Addiction Services Therapist is a member of the multidisciplinary treatment team. Major job functions include: obtaining psycho-social assessments; conducting family/group therapy on assigned cases; complete documentation per hospital policy and procedure; case coordination; coordinate discharge planning; assist in developing and maintaining community resource information; performs marketing functions as assigned; assists in the maintenance of the program environment in compliance with policies of safety and security; ensures good clinical judgment to ensure optimum patient care; participate in continuing education activities.



The following are job requirements for the position Addiction Services Therapist t:

Master's Degree from an accredited school in the Behavioral Sciences; three (3) years post Master's experience or significantly compensating prior experience, inclusive of Family Systems background, group therapy, case coordination activities, and specific treatment relate to adults in acute care settings. Must meet State Law regulations for credentialing clinicians. Must be licensed and under supervision.

To provide family and group therapy on assigned cases, including obtaining psychosocial assessment information, referral source contact, case coordination and discharge planning
Obtains psychosocial assessment information, EDO assessments, and completes report within 72 hours, including weekends and holidays
Contacts family within 24 hours of admission
Assures family therapy is conducted one time weekly, unless otherwise indicated
Assures all case coordination is completed and documented in chart
Follows up with family on all missed sessions
Assures all needed releases/consents are obtained from patient and/or family
Ensures that a preliminary discharge plan is formulated with the treatment team and family, as an integral part of treatment
Ensures that therapies conducted are consistent with the goals and philosophies of the program, and supportive of each patient's individual treatment plan
Conducts Eating Disorder Support Group weekly
Documents appropriate assessments and notes in patient chart as per hospital policy and procedure
Charting is clear, concise and reflects weekly treatment plan goals/objective movement on a timely basis
Documents discharge planning one (1) time weekly, at a minimum
Completes and signs the Discharge Contract
Assures there is a clinical therapy note in chart one (1) time weekly, if not conducting family therapy
Completes group notes in a timely manner
Completes all treatment plans in a timely manner, and includes all required clinical services objectives
Serves as Case Manager for assigned cases
Take responsibility for coordinating the treatment team process
Schedule and coordinate outside resources who wish to attend the treatment team meeting with physician approval
Check the treatment plan for specific and measurable goals for the patient to achieve. These should be short and long term goals
Check for documentation of discharge planning in the initial treatment plan and that the discharge planning is updated in each meeting
Documents referrals that are made for services not provided by the facility
Assure that changes in diagnosis, legal status, medical issues, identified problems, and discharge plans be discussed in each meeting
Participates and communicate with multidisciplinary treatment team
Attends all team meetings on assigned patients
Informs the treatment team of all details related to assigned patients in a clear and concise manner
Completes clinical service requirements on Treatment planning forms
Assists in developing and maintaining community resource information
Demonstrates knowledge about community resources for program population
Abides by hospital policy in making referrals to community resources
Maintains positive working relationships with other professionals within the community, including referral sources to the hospital
To assist in the maintenance of the program environment in compliance with policies of safety and security, and to exercise good clinical judgment to ensure optimum patient care
Takes a leadership role in responding to patient or family crisis and/or potential AMA discharges, referring to patient/family advocate when appropriate
Provides consultation to staff in regards to handling difficult patients, families, and eating disorder education
Documents and communicates breaks in safety and security accurately
Performs other duties, as assigned by Chief Clinical Officer or Associate Director of Clinical Services
Accepts and responds professionally to supervision
To maintain and exhibit knowledge related to the treatment of children and adolescents who have eating disorder issues
Exhibits knowledge of child and adolescent development
Exhibits knowledge and skill related to group therapy with children and adolescents
Exhibits knowledge of realistic expectations for children/adolescents, as well as understanding the dynamics of a family with a child/adolescent member
Serves as a clinical voice related to specific clinical unit needs
Seeks out information related to brief acute therapies
Assists Marketing Department in providing presentations regarding eating disorders to schools, community agencies, therapists, etc.
