Palo Verde Behavioral Health (PVBH) is a 84 bed  private psychiatric facility providing inpatient and intensive outpatient psychiatric services through treatment programs tailored to our patients needs for the purpose of creating positive outcomes. PVBH will build on the excellent skills of our current Psychiatrists, licensed registered nurses, clinical social workers and licensed professional counselors to support these programs.

Position Description: The Staff Nurse and the Charge Nurse provide safe and effective nursing care in the assigned area(s) in a manner consistent with the philosophy and objectives of the Hospital and within the framework of established policies and procedures of the Department of Nursing Services.  As a key team member in the clinical area, the Staff Nurse and Charge Nurse promote a climate of effective performance and learning through role modeling, willingness to learn and teach, and availability to patients and staff. 

The Charge Nurse organizes, manages and coordinates unit nursing services on assigned shift as delegated by Nursing Administration.

Sign-on Bonus of $2500 is offered to RN's who are hired as FT status.

 

Job Requirements

::

JOB REQUIREMENTS:

Education:            Graduate of accredited School of Nursing or baccalaureate program required; Current licensure as a Registered nurse

                                 thorough the State of Arizona.

Experience:  

  • Clinical experience in a psychiatric setting preferred;
  • Charge Nurse:  Minimum one (1) year in psychiatric nursing or one (1) year experience as a Charge Nurse;

Knowledge/Skills:              

  • Must have ability to communicate effectively.
  • Charge Nurse - Must be able to demonstrate leadership ability.
  • Must be able to perform concentrated and/or complex mental activity with frequent involvement in complex and/or highly technical situations;
  • Must be able to work successfully under highly stressful conditions;
  • Must be able to make sound, independent judgments based on scientific and/or ethical principles;
  • Must be able to comprehend and perform oral and written instructions and procedures;
  • Must be able to collaborate with other multidisciplinary team members in an appropriate fashion;
  • Must be capable of adapting to varying workloads an work assignments on a constant basis;
  • Must have effective comprehensive reading skills, strong communication skills, written and verbal.
  • Must have math skills to enable calculation of drug dosages and infusion rates.
  • Must possess a valid Arizona Drivers License in order to transport patients and/or staff, or drive hospital vehicles.

MINIMUM REQUIREMENTS OF THE POSITON:            

  • Must be able to demonstrate special training, knowledge and skills specific to age groups, as well as job and/or program specific competency within the first three (3) months of training, and annually thereafter.
  • Must be certified in Cardiopulmonary Resuscitation upon hire and bi-annually thereafter.
  • Must possess or obtain  CPI within 30 days of hire
  • Must possess an Arizona Fingerprint Clearance Card or the ability to obtain
  • Must complete all required mandatory inservices annually.
  • Must be tested for Tuberculosis with a PPD skin test or chest x-ray upon hire; PPD skin test required annually or chest x-ray tri-annually thereafter.
  • Must be certified in CPI training within the first three (3) months of hire and annually thereafter.

SUPERVISION EXERCISED BY CHARGE NURSE:          

Staff Nurses, Nurse Interns, LPN’s, and Care Specialists.

SUPERVISON EXERCISED BY STAFF NURSE:

Nurse Interns, LPN’s, and Care Specialists.

SUPERVISION RECEIVED:         CNO, Clinical Director, Program Director and/or Coordinator, House/Nursing Supervisor.

 

GENERAL WORKING ENVIRONMENT:              Working conditions for all employees are as follows, but not limited to a psychiatric hospital setting; some risk involved in the event of aggressive patient(s); work load may include day, evening, night, weekend and/or holiday shifts.  The Staff Nurse/Charge Nurse is occasionally exposed to outdoor weather conditions, toxic or caustic chemicals, blood borne pathogens, and loud noise levels. 

Mental demands described herein are representative of those which must be met by an employee to successfully perform the essential functions of the job, but are not limited to the ability to remain calm in a stressful environment; and the emotional stability, physical stamina and agility to handle stress and respond quickly and effectively to emergency situations.

 

 

PHYSICAL REQUIREMENTS:    Physical requirements include, but are not limited to the ability to communicate effectively with patients, their families, staff and others; the ability to effectively utilize communication equipment; the physical agility to manage patients (ambulatory, non-ambulatory and physically aggressive); the ability to read (i.e. patient charts, written communication, regulations, written policies and procedures, etc.); and the ability to write (i.e. manual charting, written communication, etc.).

  • Must occasionally utilize physical ability for walking, fingering or manual dexterity, lifting/exerting force up to 25 lbs, reaching or stretching, crouching or stooping, and smelling.
  • Must frequently utilize physical ability for standing and see with correction for color discrimination and depth perception.
  • Must regularly utilize physical ability for sitting, speaking, hearting, and seeing with correction for close, distance and peripheral vision, and focusing ability.
  • Mental demands include but are not limited to emotional stability, physical stamina and agility to handle stress and respond quickly and effectively to emergency situations.

 

EQUIPMENT/MATERIALS USED:  Telephone, computer, tape player, copier, fax machine, video player, small hand tools and writing instruments, as well as all clinical equipment, including but not limited to, syringes, blood pressure equipment, etc.

PRIMARY CRITERIA/RESPONSIBILITIES:

  1. Completes an assessment for each patient by performing a thorough mental, physical, nutritional and functional assessment on admission, and as needed, based upon patient’s condition.                                                                  
  2. Assesses patient’s psychosocial status, and patient/family learning needs and discharge criteria.             
  3. Assesses for pain, utilizes pain scale; documents accordingly.                                                                                                
  4. Utilizes appropriate observation level for patient: q15 minute checks, visual observation, and/or 1:1 or elopement precautions.

 

PRIMARY CRITERIA/RESPONSIBILITIES - Continued:                                                 

  1. Completes plan of care; formulates a nursing diagnosis based upon assessment findings.  Reviews plan of care daily and modifies as patient’s condition changes.                                                                                                                                                      
  2. Organizes and directs care of patients, setting priorities and assigning most skilled staff to care for most acutely ill patients.  
  3. Provides/coordinates education of patients based on assessment of learning needs and readiness.                     
  4. Provides care for patient’s with: Depression/Suicidal impulses, Mania, Delusions/Hallucinations, Paranoia, Anxiety, Aggressive/Assaultive Behavior, Chemical Dependency Issues/Detox, Personality Disorders, i.e. Borderline/Sociopathic, Eating Disorders, Dissociate Identity Disorder/PTSD, Dementia/Alzheimer’s, and medical problems such as hypertension, headache, diabetes, URI, UTI, heart disease, thyroid disorders, and acute or chronic infection.                                                                                                                                
  5. Intervenes in managing complex patient care situations utilizing departmental policies and procedures that promote most facilitative alternatives.  Prioritizes interventions in rapidly changing patient situations.
  6. Assesses patient’s response to interventions; communicates these findings to other team members.      
  7. Sets priorities appropriately utilizing data from Nursing Assessment.  Adapts procedures to individual patient needs while maintaining the integrity of the principles involved.                                                                                                                                          
  8. Documents education about medications, disease symptom identification, social skills, stress management, and impulse control regulation.                                                                                                                                                                                                     
  9. Is available for 1:1 interactions with patients as needed and shares in providing intervention and/or management of inappropriate behavior.                                                                                                                                                                                     
  10. Anticipates emergent situations and intervenes early and appropriately.                                                                               
  11. Identifies patients at risk for injury due to falls or behavior, coordinates the efforts of the team to prevent injury, implements Fall Precautions, and communicates to others the potential risk of injury.                                           
  12. Demonstrates effective management of assaultive behavior and appropriate use of Seclusion/Time Out.
  13. Demonstrates utilization of less restrictive measure prior to Seclusion and Restraint: 1:1: diversional activities, groups, prn medications, peer support, and/or  decreased stimulation.                                                                                                                                  
  14. Demonstrates use of application of restraints; monitors patients in restraints; provides fluids, food, ROM and bathroom.  Assesses and assists with discharge criteria.                                                                                                                                                   
  15. Demonstrates competence in CPI.                                                                                                                                           
  16. Utilizes observational skills to provide constructive feedback and input to patients.                                             
  17. Observes signs and symptoms presented by patients and acts appropriately, effectively and in a timely fashion.                                       
  18. Reports pertinent information to the appropriate discipline in a manner consistent with the acuity of the situation.                
  19. Completes daily documentation of patient’s condition and care legibly in Nursing Notes.                                   
  20. Completes charting on all assigned patients, in progress notes and all pertinent graphics.                                   
  21. Completes pertinent status reports by end of shift (i.e. incident, suicide, AWOL, infection control and acuity).                       
  22. Plans work to facilitate completion of assigned duties during shift.                                                                                     
  23. Completes work left from previous shifts and reports incomplete assignments to ensure continuity of care; finishes patient care responsibilities.                                                                                                                                                                          
  24. Establishes and maintains a therapeutic alliance with patients by assisting the patient to use the therapeutic milieu.                 
  25. Is proficient in special procedures including, but not limited to collecting laboratory specimens, glucometer, 02 Administration, dressing changes, urinary catheters, and insulin dosage calculation.                                                              
  26. Ensures that lab results and other test results are posted upon receipt.  Ensures that physician orders are affected within 30 minutes and transcribed within one hour.                                                                                                
  27. Receives and reports abnormal laboratory values to physicians; documents accordingly.
  28. Actively attends and encourages participation of patients in milieu activities.                                                                  
  29. Respects patient confidentiality in and outside of group setting and uses discretion when discussing patient information.     
  30. Transcribes and administers medications and treatments accurately.  Documents correctly and legible on medication record.  Practices the “5 Rights” of medication administration.                                                                                                                                     
  31. Ensures that medication consents are obtained and signed prior to administering psychotropic medications.                          
  32. Monitors patient for adverse drug reactions and reports any adverse drug reactions immediately to physician or pharmacist.  Knows side effects, normal dosage and contraindications of all medications given.                                                                                
  33. Demonstrates responsibility for proper safekeeping, monitoring, and accurate recording of all controlled medications kept on the unit.                                                                                                                                                                                                     
  34. Acts as a role model in demonstrating nursing practice and procedures.                                                                              
  35. Demonstrates a thorough knowledge and adherence to Hospital policies and procedures and assists with interpreting Hospital policies.                                                                                                                                                                                                     
  36. Uses good nursing judgment and needs minimal supervision, but able to recognize when needs to confer with doctor, nursing or supervisor, or program director prior to making decisions.                                                                                       
  37. Provides thorough unit orientation to new staff.  Utilizes a manner that is friendly and welcoming.                   
  38. Staff Nurse functions as acting Charge Nurse when assigned.  Supervises in a fair manner and provides direction as needed.  
  39. At beginning of shift, determines acuity.  Does not over/under estimate staffing needs.  Conveys shortage or overabundance of staff in a timely manner to allow for best utilization of staff.                                                                                       
  40. Continuously monitors acuity of unit.  Keeps supervisor informed of unusual events, conflicts, and/or patients in crisis.       
  41. Formulates staff assignments.  Ensures that team members are competent to perform tasks prior to delegation.                                      
  42. Ensures staff complete assigned duties and patient care responsibilities during the assigned shift.  Informs staff of policy changes and actively promotes strict adherence to all policies and procedures.                                       
  43. Regularly reviews work in progress, ensuring that all treatments, medications, and diagnostic tests ordered by the physician are expeditiously carried out and recorded.                                                                                                                                                           
  44. Provides shift report providing all pertinent information.  Uses concise and meaningful language.  Includes patient’s name, diagnosis, physician, precautions, level of precautions, and discharge status, if known.                                                                        
  45. Consistently monitors and documents staff performance.  Uses appropriate disciplinary measures.                     
  46. Participates in hospital-wide and unit specific performance improvement activities.
  47. Charge Nurse trained as a Qualified RN within one month of hire as a charge nurse.

  1. Attendance:  Consistently reports to work on time and is ready to work at start of shift.  Appropriately utilizes personal days/sick time.  Clocks in/out per policy and procedure; begins/returns from meal periods and rest breaks within established policy time frame.
  2. General Safety:  Follows all departmental and hospital safety, security and infection control policies.
  3. Self-Development:  Maintains required licensure/certification for position; renews by expiration date; Maintains CPI training; renews by expiration date.  Gains additional formal qualifications beyond the minimum requirements of the job; as well as additional job duties and skills.  Follows up on any personal development plan.  Maintains current knowledge of policies and procedures, protocols, and practices.  Attends inservice programs.
  4. Professional Appearance and Attitude:  Complies with the Personal Appearance policy.  Consistently demonstrates a self-directed, mature and disciplined approach to completing work duties.  Demonstrates emotional capacity to perform within position responsibilities.  Maintains and respects patient and employee confidentiality.  Follows proper lines of authority and organizational structure.
