Job Description

Stonington Institute is committed to providing quality substance abuse treatment options for adults in Southeastern Connecticut and surrounding areas.  We offer medical detox as well as residential addictions treatment for active duty and retired military service members in our North Stonington location.  Our clinic in Groton, CT, offers partial hospitalization and intensive outpatient treatment, and is conveniently located across the street from The Inn at Trails Corner, a new 180 bed dormitory-style sober community for adult men and women who attend our outpatient day treatment programs. The Inn offers overnight accommodations, full-meal service, indoor/outdoor recreation options, a workout room, and computer center for all clients.

We accept private insurance, state-funded plans and self-pay arrangements. EOE/M/F/Disability/Vet.

The Aftercare Coordinator is primarily responsible for completing each client's aftercare treatment plan. The Coordinator identifies immediate barriers to client’s recovery and familiarizes self with appropriate referral sources within the community with the intention of maximizing each client’s potential for success beyond treatment. Establishes positive relationships with clients and maintains contact with clients, clients’ families, and other service providers in order to identify medical, clinical, social and related needs of clients after treatment. The Coordinator develops and implements client discharge plans, with particular attention paid to basic needs such as housing and immediate aftercare. Works closely with the RN, Clinician, and other members of treatment team.

 

 

Job Requirements

Bachelor’s Degree in related field is required. Experience in case management/discharge planning is essential, as is familiarity with community resources for people with substance abuse and mental illness. The ideal candidate is highly organized, computer literate, and a strong client advocate. Ability to work within a multidisciplinary team consisting of RNs, MDs, clinicians, and other members of the treatment team involved with clients’ care is necessary.
