We are currently seeking an overnightpart timeAssessment and ReferralCounselor who will be primarily responsible forconducting assessments of incoming patients and responding to requests for services and information. PRIMARY RESPONSIBILITIES 1. Inquiry calls are documented and processed in accordance with Intake policies. 2. Patients are assessed and stabilized prior to leaving hospital or admission to hospital arranged. 3. Inquiry calls are documented and processed in accordance with Intake policies. 4. Conduct assessments on individuals experiencing crisis. 5. Participate in screening patients for admission into both inpatient and outpatient hospital programs, utilizing pre-approved admissions criteria set forth by the Medical Staff.&am p;am p;am p;am p;nb sp; &am p;am p;am p;am p;nb sp; &am p;am p;am p;am p;nb sp; 6. Provide assessments of incoming patient’s mental health/chemical dependency; including precipitating events, presenting problems, treatment/substance use history, mental status and risk assessment. 7. Make a determination of level of care by an evaluation of the patient’s needs, advocating the lowest level of care consistent with those needs. 8. On call doctor is notified and assists with patient interview. 9. Admission paperwork is completed in accordance with hospital policy. 10. Intake log is completed with all information documented. 11. Follow up calls are made to those failing to show up for appointments. 12. Pre-certification is obtained in accordance with insurance requirements. 13. Establishes and maintains effective relationships with customers, practicing good standards of customer service. 14. Administration is notified of problems or significant events. 15. Safety problems are identified and corrective actions taken. 16. In house assessments are completed with treatment or referral disposition recommendations made and documented. 17. Maintain good communication with the utilization review, admitting, financial offices and other departmental staff.
