Assessment and Referral Counselor (Per Diem)


If you are seeking per diem employment in the behavioral health field and enjoy working with a wide spectrum of patients, our Assessment and Referral Counselor position is for you. The Assessment and Referral Counselor is responsible for conducting assessments of incoming patients and responding to requests for services and information.  


Duties include

Making, documenting and processing inquiry calls in accordance with Intake policies,

Conducting assessments on individuals experiencing crises,

Participating in screening patients for admission into both inpatient and outpatient hospital programs, utilizing pre-approved admissions criteria set forth by the Medical Staff,

Providing assessments of incoming patient’s mental health/chemical dependency; including precipitating events, presenting problems, treatment/substance use history, mental status and risk assessment.

Making a determination of level of care by an evaluation of the patient’s needs, advocating the lowest level of care consistent with those needs.

Completing admission paperwork.

Obtaining pre-certification in accordance with insurance requirements.

Completing and documenting In house assessments with treatment or referral disposition recommendations,

Maintaining good communication with the utilization review, admitting, financial offices and other departmental staff.

Job Requirements

Education: Registered Nurse, master’s degree in counseling, psychology, social work or related field or a bachelor’s degree in psychology, sociology, or related field with a current NJ state crisis intervention screening license.

Experience: Two or more years’ experience in intake assessment activities within an acute psychiatric setting. A minimum of two (2) years’ experience in a psychiatric hospital setting including customer contact

Licensure: Licensed to practice as an RN, LSW, LCSW or LPC, preferred.

Additional Requirements: A working knowledge of the DSM-IV and the Mental Status Exam. A working knowledge of the Addiction and Recovery field.
