Primary Responsibilities:
  • Making outbound calls to assess members' current enrollment eligibility for program
  • Identifying and entering current provider information
  • Providing patient education on benefits of KRS / KCP Program
  • Interacting with Clinical Managers on challenging cases
  • Coordinating care for members by assigning and assisting RNs
  • Maintaining metric expectations for call contacts / enrollments / engagements / quality
  • Documenting and tracking findings
  • Requesting clinical data and data entry function as clinical support
  • Other duties as requested by management
Required Qualifications:
  • High School Diploma / GED or higher
  • Experience using a Windows PC and learning new application easily
  • Experience using Microsoft Word (create / edit / send / save documents) and Microsoft Outlook (send / receive emails)
  • Customer Service experience
  • Ability to work flex shifts, including afternoons, evenings, and weekends
  • Bilingual English and Spanish
Preferred Qualifications:
  • Experience in a hospital, physician's office or medical clinic environment
  • Experience in Chronic Kidney Disease / Dialysis Case Management or clinic experience 
  • Telephonic experience
  • Experience working in Medicaid and / or Medicare health care and insurance industry, including regulatory and compliance requirements
  • Renal background preferred
  • Experience with Microsoft Excel
  • Experience with EMR (Electronic Medical Records)
