Job Description

Under the direction of the Practice Manager, the Care Navigator will coordinate mental health services provided by HealthLinkNow (HLN), a telemedicine service provider throughout multiple states, and become proficient with all elements of the HealthLinkNow health technology systems including the Telemedicine system, electronic health record system, online billing and scheduling systems. Care Navigator will act as an advocate for patients and coordinate care between HLN psychiatrists, therapists and entities that refer the patients to HLN.::

Job Requirements

Primary Duties:

Provide holisticand comprehensive care navigatorservices toall clients including:intakeassessment,benefitassessment,.

Maintain proficiency with online billing system and develop reports required to assure correct billing.

Work with Staff accountant on any required reports in the HLN Electronic Health Record (EHR) system.

Work collaboratively and maintain optimal communication with all HLN staff, HLN physicians, referring physicians and their staff, all health entities and key stakeholders including Medicaid/Medicare staff in multiple states, to assure HLN services are in compliance with state and federal regulations.

Maintain proficiency as trainer for patients, external clinic staff and when necessary internal HLN staff to use HLN health technologies effectively.

Assist patients to navigate all HLN health technology systems including but not limited to Telemedicine systems, electronic health record, and online health education systems.

Provide all required documentationin a timelymanne,which may include client follow-up,outcomeevaluation,clientcontactsheets,and evaluations.

Alert HLN psychiatrists and/or therapists of any difficulties patients may encounter with treatment adherence.

Work directly with patients to assess compliance with HLN psychiatrists and /or therapist.

Act as liaison between patients, providers, internal and external stakeholders as required to support optimal health outcomes for patients.

Maintain strict adherence to HIPAA laws and regulations at all times.

Adhere to polic,procedures and theprofessionalcode ofethics.

Coordinate efficient management of referrals and care coordination between referring entities and HLN providers.

Adhere to CMS regulations for health services at all times.

Maintain proficiency in HLN telemedicine systems and work with HLN Technology Trainer and assure effective training and ongoing support for Primary Care Physicians and their staff.

Collect data and all reporting information as required and as directed by Management.

Implement and track quality measures under direction of Management.

Maintain proficiency using the HealthLinkNow Electronic Health Record system (EHR) and become an EHR “super user” who is capable of training other HLN staff and/or external stakeholders.


Essential Duties and Responsibilities include the following:

Communicate frequently with patients, physicians, agency staff to ensure quality care.

Data entry for all HLN providers, clinic, hospitals and other clients.

Respond to patient and provider inquiries in a timely manner.

Participate in developing written reports as required by management.

Participate in audits and or accrediting entities as required by management.

Other duties as assigned.



Excellent interpersonal and communication skills, self-motivated, efficient, able to meet deadlines, high attention to detail.

Capacity to become proficient in use of all HLN health systems (EHR, videoconference, online cognitive behavioral therapy) and computer programs.

Healthcare/mental healthcare experience is desired but not required.

AA or Bachelor’s Degree is desired but not required.

Proficiency in MS Word/Outlook, Excel, general computer skills.

Understands and adheres to HIPAA compliance requirements.
