Texas NeuroRehab Center (TNC) provides complex medical and brain injury rehab, chemical dependency treatment for adults and neuropsychiatric treatment for youth. The capabilities of the facility as a whole allow for integrated and specialized care for children, adolescents and adults facing difficult challenges.

Texas NeuroRehab Center is currently accepting applications for Case Manager. Case Manager coordinates patient care throughout the course of treatment for patients admitted to the Child and Adolescent Neuropsychiatric Treatment Programs. Maintain communication system based on documentation and verbal exchange regarding treatment. Communicate with internal and external entities to facilitate acquisition of treatment authorization and resources. Contribute to discharge planning. Assists with authorizations related to discharge plan. Complete treatment plans within regulatory and licensing guidelines. Facilitate weekly utilization review meeting. The candidate selected will be responsible for the following:

Performs continued stay reviews, authorizations, appeals, and pre-certifications. Completes additional pre-certification paperwork as required by the funding source. Works closely with the business office and other hospital departments. Troubleshoots case management problems and implements appropriate and timely recommendations. Completes miscellaneous paperwork as needed to assist with discharge planning, acquisition of aftercare funding, and other requests. Thoroughly documents all contact with funding sources and patient in case manager notes. Exercises leadership in scheduling, coordinating, and leading treatment team in development of the Master Treatment Plan (MTP) to include participation of patient and funding source as appropriate.

Job Requirements:
The following are the job requirements for the Case Manager:

Knowledge of psychiatric patient services. Professional verbal and written skills needed for communicating the patient status, and treatment needs to payers, families, professionals, paraprofessional employees of the hospital, and members of the patients community. Able to maintain professional records and patient confidentiality and ability to maintain professional relationships with staff and patients. Knowledge of Special Treatment Procedures. Able to develop therapeutic relationships with patients and families. Skill in preparing and maintaining appropriate medical record documentation.

Education and Training
Bachelor's degree in the humanities with 3 years of experience or Master's degree with one year of experience in case management in a psychiatric setting.

Licensure and/or Certification
Current Driver’s License or State issued Id. Established licenses are to be maintained.

Three years’ experience in a similar role, requiring verbal and written communication; prioritizing and addressing multiple tasks; and working with a team approach. Experience in working with adults with children and adolescents with psychiatric diagnoses requiring treatment.

Able to sit or stand while interacting with patients, families, and staff.
Able to write/type for extended periods of time.
Able to travel between various hospital areas.
Hearing, vision, and emotional stability sufficient to perform essential job functions.
Able to use the telephone for extended periods of time.
Able to deal with stressful situations and changing priorities.
Able to sustain physical activity eight hours a day.
Able to remain on feet eight hours per day.
Able to physically restrain patients by individual or team restraint per policy.
Able to lift, carry, and lower 50 pounds without assistance.
Able to push patients in wheel chairs up and down hills and pursue running patients.
Able to transfer patients to and from wheel chairs.
Able to confront a dangerous and aggressive patient.
Able to verbally communicate so as to de-escalate a crisis situation and accurately convey patient behavior and status.
Able to visually observe so as to convey verbally or in writing patient behavior or to protect self and others from aggressive patients or other accidents.
Able to write legibly and grammatically so as to convey patient behavior and information in the patient chart.
Regular and expected attendance.
