Job Description

Cedar Hills Hospital - located just 15 minutes from downtown Portland - is an 89-bed, free-standing psychiatric hospital specializing in mental health and chemical dependency care. We provide a wide range of services and programs that offer evidence-based treatment proven to have positive outcomes for our patients. Our inpatient and outpatient services offer supportive and compassionate care through specialty programs that are tailored to meet the needs of our patients.

The Case Manager contacts external case managers/managed care organizations for certification and recertification of insurance benefits throughout the patient’s stay, and assist’s the treatment team in understanding the insurance company’s requirements for continued stay and discharge planning. Responsible for having a thorough understanding of the patient’s treatment through communication with the treatment team, advocates for the patient’s access to services during treatment team meetings and through individual physician contact, and chairs treatment team meetings and continued stay reviews as indicated.

Job Requirements

 

Education:  Master’s Degree from an accredited college or university in social work, mental health, nursing or related field preferred.

Experience:  A minimum of two (2) years of experience in a healthcare setting or managed care company, with experience in patient assessment, treatment planning, utilization management, and/or case management.

Licensure: R.N., L.M.S.W., LPC, or other healthcare – related licensure.

  Training: Successful completion of CPR 1st Aid,  Crisis Management and Service Excellence training must occur within 90 days of employment and prior to assisting in any restraining procedures. May be required to work occasional overtime and flexible hours.
