Position Summary:

MA Level - CASE MANAGER

Job Responsibilities::
Provides a comprehensive range of Case Management and social work services to a variety of patients in an inpatient or partial hospital setting. Acts as the primary contact with external providers and participates in each assigned patient’s treatment planning. Conducts initial psychosocial assessments. Ensuring assessments, treatment plans and goals reflect integration and analysis of history and clinical observations. Actively identifies obstacles to achieve a positive outcome, initiates and implements treatment plans, working with treatment teams, supervisors and managers to resolve the obstacles. Provides group and family process orientated therapies using various professional treatment modalities. Coordinates discharge planning with patients, families and outside agencies as necessary. Attends multidisciplinary team meetings, and provides input to team members regarding psychosocial problems, overall treatment objectives, and treatment and discharge planning for each patient.

Westwood Lodge Hospital is a comprehensive behavioral health facility that provides a wide range of inpatient and partial hospitalization programs and services in a private community setting. We are conveniently located in Westwood, MA. Our facility provides a safe, therapeutic environment for children, adolescents and adult patients and their families.

MINIMUM QUALIFICATIONS:

Education: Masters degree required. Masters of Social Work or other relevant Masters degree preferred. Must be license-eligible. Licensed Independent Clinical Social Worker or Licensed Mental Health Counselor preferred.

Experience: Minimum of 1-2 Years of experience. Previous Hospital or Partial Program experience preferred.

Technical Skills: Must be proficient with written and verbal communication skills. Must have knowledge of community-based and state agencies with regard to behavioral health services.

Age and Disability: Must possess knowledge and skills necessary to interact compassionately and appropriately with patients and their families on a regular basis.

Other: Professional demeanor and appearance, as well as the ability to deal tactfully with sensitive situations and maintain patient confidentiality. Must be able to interact effectively with other hospital departments to achieve desired outcome for the patient.
