Texas NeuroRehab Center (TNC) provides complex medical and brain injury rehab, chemical dependency treatment for adults and neuropsychiatric treatment for youth. The capabilities of the facility as a whole allow for integrated and specialized care for children, adolescents and adults facing difficult challenges.

Texas NeuroRehab Center is currently accepting applications for a Charge Nurse (RN). RNs provide direct patient care and medical management and assessment of the patients. A Registered Nurse ensures environmental safety and crisis management for the patient population, and directs the implementation of behavior management program to ensure consistent carry-through.

Performs hands-on nursing care by utilizing the nursing process to achieve quality outcomes for patients.

  • Ensures that medication and treatment are administered as prescribed.  Documentation reflects the care provided.
  • Assures a safe, nurturing, clean, and supportive environment.
  • Performs skills to support direct care.
  • Can organize, prioritize, and multi-task with minimal instruction.
  • Maintains clean, well-organized, and adequately stocked patient care areas.
  • Reports equipment malfunctions and maintenance issues promptly.
  • Answers and transfers phone calls.
  • Complies with all Administrative, Clinical, and Human Resources policies and procedures.
  • Possesses the specialized knowledge and clinical skills necessary to provide care for people with physical disability and chronic illness.
  • Provides direction and supervision of ancillary nursing personnel, demonstrates professional judgment, uses problem solving techniques and time-management principles, and delegates appropriately.
  • Documents nursing care and treatment progression through timely and accurate chart entries and nursing care plan updates.
  • Coordinates nursing care activities in collaboration with other members of the interdisciplinary rehabilitation team to facilitate achievement of overall goals.
  • Demonstrates effective oral and written communication skills to develop a rapport with patients, their families, and health team members, and to ensure the fulfilment of requirements for legal documentation and reimbursement.
  • Acts as a resource and a role model for nursing staff and students and participates in activities such as nursing committees and professional organizations that promote the improvement of nursing care and the advancement of professional rehabilitation nursing.
  • Encourages others to become CRRN certified, obtain advance degrees, participate on committees, and/or join professional organizations.

  • Ensure LVNs complete Nursing Care Plan updates accurately and on time.  Sign LVN Care Plan updates.
  • Ability to remain flexible and able to prioritize and adapt during emergencies.
  • Provide guidance on administering care to new patients.
  • Document the performance of nurses and rehab techs, perform evaluations and counsel nurses and rehab techs on unsatisfactory performance.
  • Meet with management to discuss personnel and administrative issues and solve problems among staff.
  • Inform staff of changes to protocol.
  • Supervises and provides direct patient care, nursing management, assessment, and crisis management.
  • Provides leadership and effective role modeling for unit staff.  
  • Ensures environmental safety and crisis management for the patient population.
  • Makes patient and task assignments to nursing staff and monitors the performance of duties. 
  • Oversees new employee orientation to the unit. 
  • Provides training and education to staff, patients, and families. 
  • Documents and oversees the nursing documentation of patient care. 
  • Ensures administrative data, e.g. admissions, discharges, shift census reports, transporting patients, LTRAX, is accurate and complete. 
  • Supervises all staff assigned to the shift. 
  • Assesses the physical, psychological, sociocultural, and spiritual dimensions of clients and their families, as well as their educational and discharge needs in order to formulate nursing diagnoses.
  • Plans nursing care while acknowledging that rehabilitation nursing is practiced within a dynamic, therapeutic, and supportive relationship that is constantly changing, as nurses and patients influence one another.
  • Implements a plan of care by providing nursing care and education directly or through ancillary personnel, as needed, to maintain and restore function and prevent complications and further loss.
  • Evaluates the nursing care that is being provided and modifies the plan, as needed, to achieve measurable goals and objectives.
  • Shares information about the disease processes underlying disabilities and teaches nursing techniques to help patients and their families develop the self-care skills necessary to move toward wellness on the illness-wellness continuum.
  • Prepares patients and their families for future self-management and decision-making responsibilities by fostering patients’ independence and goal achievement.
  • Develops goals, in collaboration with patients, their families, and the rehabilitation team, that are oriented to wellness behavior and are reality based and that encourage socialization with others, and promotes maximal independence for patients with disabilities or chronic disabling conditions.
  • Participates in the interdisciplinary team process at team conferences and other team meetings and offers input into team decision making.
  • Intervenes with team members and other healthcare professionals to ensure that the optimal opportunity for recovery is made available to the patient, the most significant member of the rehabilitation team.
  • Collaborates with team members to achieve cost-effective care by utilizing appropriate clinical measures to meet emergent physical, psychosocial, and spiritual situations.
  • Actively listens, reflects, and guides patients and their families through the stages of the grieving process to mourn the loss of abilities and roles while also "instilling hope".
  • Advocates for policies and services that promote the quality of life for individuals with disabilities and participates in activities that will positively influence the community's awareness of disabilities.
  • Contributes to a safe and therapeutic environment and supports activities that promote the patients’ return of function and prevent complications or chronic illness.
  • Intervenes on behalf of patients to ensure that medical professionals and nonmedical professionals work to maximize patients’ success when they return to work or school.
Job Requirements

Job Requirements:
Knowledge of appropriate nursing assessment, diagnoses, and interventions with patients
Knowledge of milieu management
Ability to direct subordinates in the performance of their duties
Ability to accurately administer medication and monitor side-effect
Ability to safely implement Special Treatment Procedures
Ability to develop therapeutic relationships with patients and families
Skill in preparing and maintaining appropriate medical record documentation

Education and Training
Graduate of an accredited school of nursing
BSN preferred

Licensure and/or Certification
Current State of Texas Registered Nurse License
Current CPR certification
RN,C; CRRN preferred

One year of inpatient medical surgery, rehabilitation, or psychiatric experience preferred

Able to sustain physical activity throughout scheduled shift
Able to remain on feet throughout scheduled shift
Able to physically restrain patients by individual or team restraint per policy
Able to lift, carry, and lower 50 pounds without assistance
Able to push patients in wheel chairs up and down hills and pursue running patients
Able to transfer patients to and from wheelchairs, beds, toilets, and vehicles
Able to confront a dangerous and aggressive patient
Verbal skills to communicate to deescalate a crisis situation
Writing skills to accurately convey patient behavior and status
Visual observation skills so as to convey verbally & in writing patient behavior or to protect self and others from aggressive patients or other accidents
Writing skills to include legibility and correct grammar to convey patient behavior and other information in the patient chart
Ability to assist in direct patient care from prompting through total care (i.e. bath, diapering)
Able to feed patients
Regular / required attendance
Hearing and vision sufficient to perform essential job functions
Emotional stability and ability to deal with changing priorities
