Henderson Hospital
Henderson Hospital is a member of The Valley Health System, a network of six acute care hospitals that provide care for patients throughout Southern Nevada and nearby communities. Located in the suburb of Henderson, the 120-bed hospital, which opened in 2016, offers emergency care, surgical services, including an ambulatory surgery center, cardiovascular care, women’s health and maternity services. A level II neonatal intensive care unit is slated to open by late 2017.

 

Job Description

Assumes shift responsibility and accountability for the planning and provision of direct and indirect nursing care to the patient, family, care partner and appropriate others. Coordinates multi-disciplinary team members, directs and facilitates nursing staff in obtaining safe, optimal health care outcomes. Supports activities that meet the facility vision, mission and goals. Provides professional leadership and support to the healthcare team. Oversees and guides employees that are under his/her supervision. The concepts of Patient Centered Care will provide the foundation for all nursing care. Demonstrates Service Excellence standards at all times.                                                                                                                                                                                       

 Essential Job Duties:

Provides effective leadership for the implementation and improvement of the required components of the patient centered care delivery model. (Example: Bedside report, hourly rounding, etc.).   
Takes a lower acuity assignment as needed to meet unit needs while continuing to serve as a resource and leader for patient care problems.          
Completes unit patient rounds with purpose to evaluate the patient status, perception and quality of care provided. Supports compliance with hourly rounding with purpose to promote nurse communication, improve safety, and anticipate patient needs (i.e. pain, re-positioning, and toileting). Rounds with purpose including pro-actively evaluating the environment to ensure it is patient friendly and patient safe.
Involves the patient and family psychosocial, spiritual, emotional, population specific, and diversity needs in care assessment.              
Constructs and implements the plan of care using nursing knowledge and skill. Integrates direct and indirect patient care activities related to the diagnosis and co-morbidities. 
Coordinates the implementation of the interdisciplinary plan of care for assigned patients. Develops goals, treatment plans, and discharge plans that meet individual patient/family needs.   
Collaborates with the patient’s designated care partner to meet the educational, physical, psychological, and spiritual needs of the patient.       
Actively participates in bedside report (as appropriate) to ease anxiety and address important points of care, concerns, and progress with the patient/family.             
Provides patients with caregiver name and contact information each shift.        
Provides care that is respectful of and responsive to the patient values in clinical decision making.
Supports and informs the patient and family of hospital specific components of the Patient Centered Care Delivery model. Examples may include informing the family of open medical record policy, supports open family visitation, family presence during resuscitation and other invasive procedures and promotes a quiet environment.        
Gives a charge nurse report including all relevant information to promote continuity of care and safety.          
Assumes role of team member, charge nurse or team leader based on the need and/or situation. Communicates effectively with the unit manager/director on the issues addressed, pending and the resolution. Seeks assistance on the necessary follow-up or resources needed.              
Serves as a role model and mentor to unit staff. Shares knowledge of policies with team members.            
Facilitates effective productivity through the use of personal organizational skills and time management. Promotes a positive work environment and team cohesiveness.  
Assists physicians with resolution of issues to promote prompt resolution. Demonstrates service recovery skills when addressing patient care issues.    
Completes schedule as assigned and adjusts staffing for the shift by adjusting staff to the workload and workflow to maintain worked hours within the budgeted standard.           
Utilizes any down-time appropriately to reassign and/or assist staff as dictated by patient care and unit needs.   
Collaborates with team members to develop solutions. Is accessible and interacts positively with staff and other members of the health care team.            
Acts with integrity, consistency, and respect for differing views.            
Follows communication practices that minimize risks associated with handoffs among providers, and across transitions in care utilizing the SBAR process.      
Leads or participates in the design and implementation of systems that support effective teamwork.          
Empowers contributions of others who play a role in helping patients/families achieve health goals.    
Solicits input from other team members to improve individual, as well as team, performance.              
Initiates actions to resolve conflict.
Participates in review of core measure compliance, evidence based policies and practices and facilitates consistent high compliance with evidence based practice.

Demonstrates proficiency in completing an evidence-based practice literature search.

Demonstrates quality through accurate documentation of the nursing process in a responsible, accountable manner.        
Demonstrates quality through accurate documentation of core measures and concurrent use of tools and resources to ensure high compliance is achieved (core measure checklist, double checks, etc.).       
Utilizes the results of quality improvement activities to initiate changes in nursing practice and care delivery.      
Understands and analyzes unit metrics to understand how individual practice, continuing education, and process improvement is necessary to improve the quality of practice.     
Participates in efforts to minimize cost and eliminate waste and unnecessary duplication.              
Utilizes policies, procedures, and evidence-based guidelines to improve the quality of practice.              
Participates in interdisciplinary teams to evaluate clinical care or services.         
Demonstrates competent clinical skills, critical thinking, and nursing judgment.
Assesses the qualifications and competence of unit staff providing patient care services. Makes patient care assignments considering the patient acuity/complexity, staff ability/competence, turnover, and skill mix/availability within the established guidelines. Delegates tasks to appropriate team members. Makes decisions that are safe and effective.           
Facilitates staff compliance and holds staff accountable for providing safe patient care, assessment, monitoring and documentation requirements as well as facility requirements for training and professional appearance and behaviors. Supports staff in emergent patient care situations.           
Direct staff to complete all safety checks, equipment checks and completion of orders to be completed during the shift. Adjusts staff as needed to meet patient care needs (up-size/down-size staff). Requests assistance from the manager or shift supervisor, as appropriate.      
Is proactive in planning ahead for the unit needs; anticipates staff requirements and takes necessary steps to meet the supply and demand.      
Follows facility policies and procedures to promote patient safety (i.e. safe medication administration, equipment safety practices, radiation safety, fire prevention, etc).          
Reports errors (facility and unit) to promote analysis of errors and design of prevention or system improvements.     
Utilizes appropriate patient safety strategies to prevent human errors (checklists, forced functions, software calculations, mandatory fields, etc.).         
Demonstrates effective use of technology and standardized practices to promote safety.
Navigates the patient's electronic health record to accurately enter and locate appropriate information.        
Demonstrates sound judgment to increase or decrease the frequency of data collection based upon the patient's clinical condition and data trends.
Incorporates patient data from multiple sources to identify problems and assure timely and appropriate intervention.
Ensures the confidentiality and security of patient information while advocating for and involving the patient as appropriate.            
Participates in the collection of data to support nursing research and evidence based nursing practice.
Maximizes the capabilities of the electronic clinical information system to enhance clinical decision and care planning.             
Demonstrates accountability and maturity in job performance. Supports the facility and departmental goals and objectives.
Participates in coaching and counseling of staff, staff interviews and provides feedback for hiring decisions for the unit. Completes or provides input on staff evaluations in an objective, timely manner.   
Communicates effectively and professionally with all team members. Facilitates accurate and timely communication in collaboration with the unit manager/director.             
Makes discretionary patient care decisions/interventions independently within the scope of practice (selects a course of action consistent with the patient’s needs and is accountable for decisions).  Is self-directed and decision oriented.     
Practices interdependently, utilizing the skills and observations of team members appropriately.     
Practices with respect and dignity and advocates for the patient. Follows the chain of command and supports staff with more complex situations. Escalate problems as appropriate while resolving issues at the lowest level possible.               
Facilitates problem resolution among peers and unlicensed team members when behaviors and decisions are not within the range of expected actions.    
Prioritizes and delegates care appropriately.  Demonstrates effective organizational skills and time management.            
Ensures resolution of all PYXIS discrepancies prior to shift end.             
Ensures patient assignments are made based on patient need and staff competency level. Assigns unit tasks, meal/break time and ensures completion. 
Ensures that Cerner systems accurately reflect unit census. Also ensures that physician orders are input accurately via Cerner and carried out in a timely manner.      
Provides complete End of Shift written report to unit mgr.      
Completes unit based Performance Improvement activities as assigned.            

Evaluates changes in the patient condition and reports significant findings to the physician. Involves all disciplines, patient & family to coordinate effective medical and nursing interventions including rescue to achieve positive outcomes. 

Job Requirements

Job Requirements:

Graduate of an accredited or NLN-approved RN program. BSN preferred.                                                                                                                                

Minimum of one year full time or three years part time experience in acute care specialty nursing. One to three years supervisory experience preferred.                                                                                                                                    

Must be proficient in physical assessment of the cardiovascular patient and fluent in cardiovascular anatomy and physiology.  Must be comfortable with 12 lead EKG interpretation and cardiac arrhythmias.  Knowledgeable and experienced in stress testing is preferred                                                                                                                            

Current RN license in the State of Nevada.  Current BCLS Certification.  Current ACLS Certification. Other certification as may be required by facility, specialty or department.
