Southwest Medical, part of OptumCare, is a multi-specialty group of physicians, Nurse Practitioners, and Physician Assistants consisting of over 300 providers, 21 medical health centers including six urgent care clinics, four convenient care centers, two lifestyle centers catering to older adults, an outpatient surgery center, home health, hospice and palliative care services, plus E-visits and online options via Now Clinic telemedicine technology. Southwest Medical offers patients compassion, innovation, and quality care throughout southern Nevada. Southwest Medical is headquartered in the greater Las Vegas, NV area.
 
The CAC functions as part of a team that assists with specific work with patients in clinic and home settings assisting with paperwork submission for various entities.  The CAC is responsible for making outbound calls to set up appointments for recently discharged patients, paperwork submission and to screen for any assistance needed. The CAC coordinates clerical functions for the coordination as requested.  The CAC is responsible for clear documentation and communication regarding any delegated duties and member contacts.  The CAC interacts with other team members concerning patient needs identified during the course of day-to-day activities. The CAC gathers patient data through telephonic communication, electronic medical records and provider offices to provide data to the team.  The CAC may be requested to develop and generate reports. 
 
Primary Responsibilities:
  • Verify eligibility for members
  • Assist patients with paperwork for a variety of community resources as directed
  • Function as a member of an interdisciplinary team to facilitate patient care
  • Assist with care planning activities to include gathering necessary information, performing data entry, following up with provider office staff as needed
  • Place telephone calls to patients and assisting appropriately
  • Prepare information for submission to various authorities and resources
  • Assist various programs with setting appointments, gathering documentation and providing follow-up as directed
  • Maintain accountability and promote growth within the CAC team
  • Other duties as assigned
