Position Summary:
Fremont Hospital is a 148 bed acute psychiatric hospital situated in the heart of the bay area. We are the largest freestanding psychiatric hospital in the East Bay filled with diversity and the feel of a close knit community. We currently seek an experienced Clinical Case Worker Lead to fill our open positions. The Clinical Case Worker Lead perform clinical services functions in assisting patients in meeting their biopsycho-social needs. These include enabling the patient, through therapeutic treatment skills, to achieve their optimal level of emotional health. Services provided are basic consultation, teaching, specialized clinical services for patients/families receiving care, crisis intervention, short-term family therapy, and providing information and referrals as needed.

The Clinical Case Worker Lead will be responsible for managing the milieu treatment activities offered to the patient, completing the psycho-social assessment forms for each new patient and entering specific information on the treatment plans; interfacing with the physician led treatment team; provision of psychotherapeutic groups, psycho-educational groups, and other pertinent activities to patients and their families; provide oversight and coordination of legal paperwork and Lanterman Petris Short (LPS) regulations.

The Clinical Case Worker Lead will be responsible for initiating, developing, and implementing the provision of services designed to assist the patient in a successful transition from the hospital setting to home or other placement. Discharge planning will be initiated upon admission and continually assessed with the treatment team during the development of the Master Treatment Plan. Criteria for discharge will be noted in the Treatment Plan and will be included in ongoing documentation of Treatment Plans and specific staff responsible for providing these services. On-going discharge planning documentation will be audited by the Director of Social Services.

In addition, the Clinical Case Worker Lead will be responsible for the facilitation of clinically pertinent discharge planning and case coordination; interface with Fremont Hospital Clinical Staff, with outside referral agencies, with managed care organizations to ensure that the admission, continued stay, and discharge planning is in compliance with Joint Commission (JCAHO), Center for Medicaid & Medicare Services (CMS), established Discharge Planning Policies and Procedures and Clinical Services Policies and Procedures. The Lead will be responsible for their assigned units compliance with the standards mentioned above.

The Clinical Case Worker Lead be responsible for facilitating treatment planning including the initial treatment team meetings, treatment planning updates, and specialized treatment plans. The Lead will be will act as a resource for other clinical staff and train/orient new staff and students (department orientation). The Lead will also provide monthly didactics to social services staff including evidence based treatment.
 
Requirements
Education: A Bachelor’s degree for an accredited school of Social Services, Psychology, Nursing, Social Work, or Mental Health related field required. A Master’s Degree is preferred.

Experience: Minimum 2 years direct clinical experience in a psychiatric or mental health setting and experience in patient assessment, family motivation, treatment planning and communication with external review organizations or comparable entities is preferred.

Licensure & Certification: The Master’s level may be actively pursuing and registered for professional licensure by the California Board of Behavioral Sciences preferred (MFTi or ACSW)

Additional Requirements: CPR/AED Certification must be current and presented to Human Resources upon hire. Successful completion of Fremont Hospital’s non- violent Crisis Prevention Intervention (CPI) training course within 90 days of employment and prior to assisting with preventative intervention measures. Displays leadership, ownership and dedication to superior quality care. Will oversee and sign off on all clinical activities performed by Bachelor’s degree level and interns (II).
