Job Requirements

Essential Job Duties:

Reporting to the Director of Health Information Management, the Clinical Documentation Specialist works to improve the overall quality and completeness of clinical documentation by facilitating modifications to clinical documentation through extensive interaction with physicians, nursing staff, other patient caregivers, and health information coding staff to help ensure that appropriate reimbursement is received for the level of service rendered to the patients. Manages and coordinates the day to day activities of the clinical documentation improvement area; develops, maintains, interprets, and evaluates procedures and processes to ensure that activities are carried out in an effective manner.  Responsible for reviewing medical records to identify payer population as documented upon admission and throughout the hospitalization.  Analyzes status of patient, current treatment plan and past medical history and identifies possible gaps in physician documentation.  Works with Case Management on RAC reviews, HIM and Medical Staff in facilitating appropriate documentation of care.  Provides education to physicians and other patient caregivers on regulatory/reimbursement changes.  Addresses incomplete physician documentation, compiles data on MS-DRG assignment and severity/mortality profiles by working closely with Case Managers, Coding Supervisor, and coders.  Assigns working DRG while the patient is in house.  Reviews requests for retrospective MS-DRG reviews from external and internal sources preparing appropriate responses.

 

Job Requirements:

Minimum of 5 years recent clinical experience in an acute care setting required.  Critical care of medical/surgical preferred.

Associates Degree required or Bachelors of Science Degree preferred.

Registered Nurse or International Medical Graduate (IMG).
