POSITION SUMMARY:

The Nurse Liaison receives patient referrals via phone or fax.  He/she arranges for evaluations and reviews initial payor data.  This individual secures appropriate contact information and assesses patient’s rehabilitation potential and suitability for hospitalization.  They interview patient and family to gather information and determine special needs.  They must adhere to admission criteria and present potential patients for approval at admissions meeting.  They also inform physicians of pending admission and coordinate pertinent data with Business and Admitting Office.  They represent the facility in all contracts with other health professionals and the general public in a manner which enhances the facility’s reputation.  This position summary contains the most relevant duties and responsibilities and does not exclude other assignments not mentioned.

Job Requirements

::

QUALIFICATIONS:

1.  Must have at least two (2) years experience in medical or physical rehab setting.

2.  Good organizational and liaison capabilities with well developed written and verbal communication abilities.

3.  Demonstrated public relation abilities with cooperative and assertive attitude.

4.  Working knowledge of Medicare guidelines for Rehabilitation.

5.  Must demonstrate commitment and adherence to STHS’s Compliance Program and Code of Conduct through compliance with all policies and procedures, the Code of Conduct, attendance at required training and immediately reporting suspected compliance issue(s) to the Compliance Officer.

 

 

EDUCATION / LICENSURE:

1.  Must have a current RN licensure in the State of Texas.
