The Optum, Long Term Care Clinical Team Leader is responsible for managing approximately 25-35% case load and direct supervisory responsibility for 4 to 6 Nurse Practitioners These Nurse Practitioners provide primary care to patients in long term care settings; coordinate with their PCP and facility staff to deliver high quality care on-site.
Primary Responsibilities:
  • Providing direct primary care to aging residents in long-term care facilities in collaboration with primary care physicians
  • Coordinating care by collaborating with primary care physicians, consulting physicians, facility staff and patients' families
  • Participating in care conferences with other providers and patient family members
  • Educating nursing home staff, physicians and administration
  • The Clinical Team Lead will also serve as the direct supervisor for a team of 2 - 6 Nurse Practitioners
  • Provides primary case management of chronic and acute conditions. - Reviews the work of others
  • Develops innovative approaches
  • Sought out as expert
  • Serves as a leader/ mentor
  • Coordinates, supervises and is accountable for the daily activities of business support, technical or production team or unit
  • Impact of work is most often at the team level
