Job Description

Reporting to the Director, the mental health worker/certified nurse assistant will assist in the promotion, maintenance and restoration of health of patients assigned to his/her care. Responsibilities include assisting with ADL's, promoting wellness, health/education and clearly documenting patient activities. Responsible for adult and geriatric patient's personal hygiene, ADL's, vital signs, application of ice bags, K-Pads, assist patient with TC and DB and use of incentive spirometer and providing a safe and comfortable environment for their patients. They provide emotional support to their patients and document their interventions.

 

 Essential Job Duties:

Assess vital signs, including pain, patient height and weight, I&O, and ADL or other nursing care needs on all assigned patients per policy.  Reports nursing care needs of patient and communicates to nurse.  Assesses changes in care and needs and reports to Charge Nurse.

Assessment considers age 55 or older related needs as appropriate ( i.e. risk for falls and decrease in skin integrity, etc).

Demonstrates basic knowledge of medical/surgical, psych intervention associated with patient's disease state.

Exhibits knowledge of technical skills required to implement safe nursing care and health promotion of the hospitalized patient as demonstrated through practices.

Provides quality-nursing care based on patients needs, age, plan of care including personal hygiene/dental care at the direction of the nurse.

Demonstrates proficiency in the use of equipment (i.e. EKG's, oxygen tanks, bed scales, lifts/patient handling, etc).

Identification of patient and/or significant others educational needs is reported to the Charge Nurse.

Utilizes individualized teaching techniques based on patient's needs (i.e. sensory loss, learning deficits, language barrier), ability and willingness to learn.

Documents a minimum of once per shift or when there is a change of condition.  Documentation reflects the plan of care and the activities of the patient care per physician orders.  Suggestions for the plan of care are given to the Charge Nurse.

Works closely to coordinate care given by all departments.  Supports the needs of other departments.

Provides concise and meaningful report to fellow workers, charge nurse and physicians.

Participates in unit meetings at least 50% of the time.  Updates self with staff meeting minutes, memos, policies and procedure changes.

Participates in QI/Performance Improvement activities.

Completes assigned routine unit duties (i.e. Q 15-minute rounds, ADL's, I&O, secretarial assistance, assigned 1:1 nursing functions, etc).

Submits written documentation (statement of concern, incident reports) as appropriate.

Orients new personnel to the unit.

Functions appropriately in the crisis situation as directed by the Charge Nurse.

Admission, transfer and discharge procedure are followed including accurately recording patient's valuables/belongings and contraband according to approved policies and procedures.  Ensures that belongings are moved with the patient.

Demonstrates an understanding of limitations of patients related to aging/disease process (55+).

Demonstrates the ability to apply restraints and complete necessary documentation i.e., flow sheets and q 15-minute checks.

Escorts patients for transports.

Monitors patient safety and immediately reports concerns to the charge nurse.

Ensures telemetry box is removed and returned to monitor tech when patient is discharged.

Patient Education - reports patient or other's educational needs.

ESSENTIAL KNOWLEDGE / SKILLS

Knowledge of Patient Rights.

Knowledge of physiological changes that occur with patients age 55 plus.

MINIMUM ESSENTIAL EXPERIENCE

Six months recent hospital (med/surg and/or psych) experience required.

MINIMUM ESSENTIAL EDUCATION

Assaultive Behavior Management training required within 90 days from date of hire.

REQUIRED LICENSURE / CERTIFICATIONS

Current California Certified Nurse Assistant Certificate required.

Current BLS (CPR) Certificate required.
