Position Summary:

The potential candidate will pose the below responsibilities: Serves as a positive role model for children/adolescents and adults to observe and learn socially effective values, attitudes and behaviors, and contributes to formulation and implementation of the treatment plan. Provides individualized services to include medication education, teaching daily living skills, enhances social and interpersonal skills. Travels throughout the community to provide mental health services in the home and to act as a community liaison between the individuals we serve and the service providers in the community. 1. Interacts with individual in accordance with the treatment plan and the therapeutic process. 2. Maintains therapeutic relationships and accompanies individuals to appointments and outings as necessary. Exercises appropriate intervention in accordance with the least restrictive alternative to protect individual from harming themselves or others. 3. Notifies Program Supervisor of any unusual events, medical problems or potential hazards. 4. Travels frequently throughout the community performing the duties of a community liaison. 5. Attends team meetings and staffing as required. 6. Records progress notes and progress reports of behavior and include specific incidents, and conditions such as suicidal thoughts and actions, aggressive behavior, special checks, restrictions and unusual sleep patterns. 7. Works closely with individual and treatment team to ensure treatment plan is being followed.
 
Requirements
Qualifications: To perform this job successfully, an individual must be able to perform each primary duty satisfactorily. The requirements listed below are representative of the knowledge, skill, and/or ability required. Reasonable accommodations may be made to enable individuals with disabilities, who are otherwise qualified, to perform the primary functions. Education: Master’s Degree in a human services field. Human Services field includes Social Work, Psychology, Sociology and Counseling; LMHP eligible. Licensure/ Certification: One or more years experience providing direct clinical services to children and adolescents and adults with mental health illness. Experience in Crisis Intervention setting preferred. Physical Requirements: Must be able to remain in a stationary position 20% of the time. Works in the community going from one place to another by car and then into houses and other facilities. Constantly operates a computer and other office productivity machinery, such as a calculator, copy machine, and computer printer. The person in this position frequently communicates with clients. Must be able to exchange accurate information in these situations. Must be able to work with clients and judge their mental health status. Must be able to see client appearance to see if they are taking care of themselves. Must be able to carry lap top and bag with items in it weighing up to 10 pounds. Works in the community going from car to inside other people’s houses and other facilities within the community.
