Children’s Rehabilitative Services (CRS) is an Arizona Medicaid program that provides integrated care to members with CRS-qualifying conditions.  Services include specialty care, primary care and behavioral health care. CRS members are able to receive care in the community or in multispecialty interdisciplinary clinics (MSICs) across the state.   
 
We have an excellent opportunity available for you to bring your extensive experience and join our company as a Telephonic RN Case Manager, working in our Phoenix office. As a part of the team, you will be responsible for providing specific clinical, complex case management activities across the continuum of care from assessing and planning to implementing, coordinating, monitoring and evaluating. This position provides telephonic case management and education in support of beneficiaries and their families, who range in age to infants to older adults, and for our Developmental Disabled (DD) members. Members in your care have been identified with special health care needs both medically and behaviorally.
 
Primary Responsibilities:   
  • Educating and supporting the individual through telephonic assessment and expert clinical case management interventions
  • Assess and interprets beneficiary needs, gaps and requirements related to his / her condition
  • Create care plans / addressing gaps or barriers in care
  • Providing patient education to assist with self-management
  • Interacting with Medical Directors and community partners on challenging cases
  • Coordinating integrated care for members
  • Making referrals to providers and community resources
  • Encouraging members to make healthy lifestyle changes
  • Documenting and tracking findings

This is an office based position located at 1 East Washington St, Phoenix, Arizona.

Required Qualifications:   

  • Current licensure as an RN in the State of Arizona
  • 2+ years Behavioral Health experience
  • 2+ years Medicaid, managed care or health plan experience
  • A working knowledge of a special needs population
  • Intermediate experience with Microsoft Word, including Excel, with the ability to navigate a Windows environment

Preferred Qualifications:   

  • Experience with provider services, billing and enrollment, member services, and claim processes and procedures
  • Clinical pediatric experience
  • Mental health / behavioral health experience; working with Arizona’s Regional Behavioral Health Authorities (RBHA)
