Job Requirements

The Discharge Planner is responsible for coordination of discharge planning services within the Utilization Review Department. These duties include but are not limited to daily audit to ensure effectiveness of clinical plans, verification of communication with appropriate supports and partners, direct discharge planning and case coordination assignments, provides reports and develops discharge process improvement as directed by the Director of Utilization Review.

EDUCATION and/or EXPERIENCE:  RN, LPN, and/or Bachelor’s Degree in a clinical field preferred.  An equivalent combination of education and experience may substitute.

REASONING ABILITY:  Ability to define problems, collect data, establish facts and draw valid conclusions.

LANGUAGE SKILLS:  Must be able to express or exchange ideas by means of spoken and written word.
