Millwood Hospital  is seeking a dynamic and talented VAN driver

This position provides transportation of patients to and from the facility providing continuous observation, interaction, and role modeling to the patients while maintaining safety at all times.

May be called upon to serve as Program Specialist/MHT as needed, providing highest level of care/safety with patients.

Key Responsibilities include:

1.       Provide transportation of patients to and from facility.

2.       Interact with multidisciplinary team to provide input related to patient management while in transport. Hands off communication is completed both pre and post trip as indicated.

3.      Responsible for the van log ensuring that the address and phone numbers for patients are

         updated.

4.       Report any vehicle safety or operational issues to Millwood Hospital Director of Plant Operations.

5.       Appropriately document any significant incidents that occur related to transportation, relay such information to Director.

6.       Complete vehicle safety inspection pre trip and post trip and report any problems or damages.

7.       Report any vehicle accidents immediately to Director.

8.       Maintain a clean, safe environment in the vehicle at all times.

9.       Refill vehicle with gasoline daily after route is completed and prior to loading patients.

10.     Maintain communication with guardian of patients in terms or expected pick up and drop off times.

11.    Demonstrate responsibility and sound judgment regarding patient safety.

12.    Adhere to facility standards concerning conduct, dress, attendance and punctuality.

13.    Support facility-wide quality/performance improvement goals and objectives.

14.    Maintain confidentiality of facility employees and patient information.

 

Job Requirements

QUALIFICATIONS

Education:   High school graduate, or equivalent preferred. 

Experience: Minimum of one (1) year working with children and/or adolescents preferred. Experience driving passenger van preferred.

Licensure:  Maintains a valid, class specific driver’s license applicable to the transportation 

of patients.

Additional Requirements:  Must be 18 years or older and have a good driving record as reported by the Department of Public Safety.  CPI and CPR per New Hire policy must be completed prior to providing direct care services. May be required to work occasional overtime and flexible hours.
