In this role, you will conduct in-home health assessments on enrolled Medicare Advantage members within a geographic area of responsibility. While the average visit is 45-60 minutes, the nature of the House Calls visit can vary and could include performing an annual health assessment, a post discharge visit, or visiting more complex members more than once per year.

Primary Responsibilities:

  • Conduct annual physical exam & wellness assessments within the clinical setting with strong emphasis on senior conditions
  • Confirm, disagree or mark resolved chronic conditions from patient-specific lists of historical conditions; confirm plan of care developed for patients seen in the primary care clinic
  • Discuss and provide health coaching and health education to engage patients to be an active participant in maintaining good health
  • Provide guidance to patient questions about health care plans and care issues
  • Document senior assessment in Lifeprint assessment form and plan of care in a stand-alone medical record
  • Document collaborative guidance from the physician as needed
  • Emphasize and review preventive measures per evidence-based medicine
  • Review medications for polypharmacy compliance and educate patients on medications
  • Collaborative review of chronic preventative measures for seniors for quality review
  • Provide health screenings as indicated by a Focus on Care Screening
  • Recommendation report that is provided for each patient
  • Coordinates care with the patient
