As a Pediatric Field Based and Telephonic Case Manager, you'll interact with medically complex children. You may also act as an intermediary between providers and members - serving in numerous roles, such as educator, evaluator, service coordinator, community resource researcher and more. The result?  Assisting Medically Complex Children and providing family support with expert symptom management, and care coordination. 
 
If you are located near Orlando, Florida, you will have the flexibility to telecommute* as you take on some tough challenges.
 
Primary Responsibilities:
  • Visit members in their homes and/or other settings, including community centers, hospitals or providers' offices
  • Provide a complete continuum of quality care through close communication with members via in-person or on-phone interaction
  • Support members with condition education, medication reviews and plan of care development
  • Assist members with the transition from a care facility back to their home
  • Making outbound calls to assess members' current health status
  • Working with the complex care needs
  • Identifying gaps or barriers in treatment plans
  • Interacting with Medical Directors on challenging case
  • Making referrals to outside sources
  • Coordinating services as needed (home health, DME, etc.)
  • Educating members on disease processes
  • Documenting and tracking findings
  • Provide monthly updates to Agency for Health Care Administration

This role involves approximately 25% travel in the greater Orlando, FL area and you’ll need to be comfortable dealing with a full range of in-home and facility-based environments.

Required Qualifications:

  • Current, unrestricted RN license in the state of Florida
  • Access to reliable transportation
  • 2+ years clinical experience in NICU and/or Pediatrics, hospital, acute care, home health/hospice, direct care or case management
  • Experience working with children with chronic illness and complex care needs
  • Proficiency with Microsoft Office Suite, with the ability to navigate a Windows environment, excel spreadsheet, experience with email, internet research, use of online calendars and other software applications
  • Candidates must obtain a CCM within 30 months of employment if you do not already have one
  • Must be able to travel in the greater Orlando, FL area at least 25% of the time

Preferred Qualifications:

  • BSN
  • Case management experience

  • Certification in Case Management

  • Home care/field based case management

  • Experience working with the needs of vulnerable populations (pediatric members) who have chronic or complex bio-psychosocial needs

  • Bilingual Spanish / English

  • Experience working with pediatrics/NICU as a specialty

  • Experience in Home & Community based, Hospice, Home Health or Long Term Care services delivery

If the hired individual resides in Florida (office based or telecommuting) this position requires the AHCA Level II background check (fingerprinting) by the State of Florida for all clinicians that have direct face to face contact with members OR employees who will have access to confidential patient data and will require renewal every five years.
