The Heath Systems Change Specialist provides tobacco cessation and chronic disease intervention related training, technical assistance, and resources to health care providers and health delivery systems.  The primary goals of the Health Systems Change Program is to improve the ability of health care providers to systematically and routinely identify tobacco users and refer patients to appropriate resources, such as the New Mexico Cessation Services.  The Specialist conducts daily field-based outreach with health professionals including clinical providers and health administration staff in identified clinic and hospital-based health systems throughout a multi - county territory in the state of New Mexico. This is a home - based field position and requires a significant amount of travel within the territory, including overnight stays. Applicant must be abstinent from tobacco, have reliable, daily access to a car, a valid driver’s license, and maintain a clean driving record. 

 

Primary Responsibilities:

  • Conduct proactive outreach with health and behavioral health care organizations and providers to deliver health systems change consultation, including technical assistance and training
  • Conduct organizational assessments to identify needs and make recommendations regarding next steps
  • In partnership with organizations, develop plans and support implementation process of tobacco use identification and treatment health system changes
  • Report and track all interactions with health care organizations and health care providers
  • Develop and execute weekly and quarterly work plans to ensure organizations in a territory receive adequate training and technical assistance
  • Participate in ongoing training, coaching and skill building activities. Remain updated on tobacco cessation, electronic health record, health policy and program topics, and chronic disease prevention and control programs and policies
