The Oaks at La Paloma is currently seeking an HIM/Medical Records Clerk for their inpatient residential treatment facility located in Memphis, Tennessee.

Position Summary

The HIM/Medical Records Clerk will assume responsibility for compiling, processing, and maintaining patient medical records in a way that is consistent with regulatory healthcare requirements.

Key Responsibilities

  • Ensures records are complete before they are closed.

  • Maintains all closed medical records

  • Responds to requests for medical records

  • Pulls records as requested by staff including daily pulls for appointments

  • Files documentation presented by clinical staff

  • With appropriate, signed request for information, copies records upon appropriate request for billing, legal services, or medical records requests
  • Request historical records from other organizations according to policy.

  • Knows standards for medical records maintenance and ensures compliance with all Joint Commission and regulatory standards.

  • Tracks information needed to meet medical record standards, auditing the records and ensuring the records are complete before they are closed.

  • Keeps supervisor informed of problems or issues; monitors supplies needed.

Job Requirements


  • Medical Records or related certification required; Associates Degree related field preferred.

  • Minimum of 2 years of experience working with medical records.

  • Certificate or experience with Medical Coding preferred.

  • Knowledge of Joint Commission and regulatory standards.

  • Must demonstrate high level of interpersonal skills.

  • Excellent verbal and written communication skills required.

  • Ability to pay continual attention to detail.

  • Excellent organizational skills with the ability to prioritize workload.
