The Home Health Clinical Team Coordinator is responsible for staffing all new referrals, scheduling of patient visits daily, end of day schedules and ensuring all weekend schedules are complete and reviewing schedules daily with field staff.  Assists Clinical Team Manager in maintaining daily contact with field staff, communicating schedule changes to field staff when a change occurs.
 
Primary Responsibilities:
  • Responsible for scheduling all physician orders, new referrals and schedules appropriate field staff member on a daily basis
  • Faxes or scans patient information to field staff as needed
  • Schedules any changes or corrections per MD orders in EMR
  • Ensures that the new referral is staffed with an RN and any other staff members needed to make sure what was ordered by physician is scheduled correctly
  • Schedules and plots all tasks needed for that patient from beginning to end: supervisory visits, recertification visits, discharge visits, transfers, and post-hospital visits
  • Contacts field staff regarding missing outstanding documentation on a weekly basis
  • Updates schedules frequently to remain in compliance with the current physician order
  • Retrieves lab results from previous day
  • Communicates all new orders and new referrals with contract therapy staff
  • Assists CTM’s with contacting field staff or relaying information in reference to patient care
  • Communicates frequently with management on unassigned patient visits to ensure that all patients are seen within the time frame needed per physician orders
  • Responsible to input transfer tasks once notified patient hospitalized on appropriate field staff member
  • Prepares weekend staff-reviews all patient schedules ensuring field staff is aware of current orders or any changes in their schedules
  • Demonstrates exceptional customer service not only to patients, but to all staff and family members
  • Answering of phones with quality customer service
  • Follows up with CTM’s, case managers, DME dept in reference to new orders, patients information to ensure patient’s needs are addressed
  • Updates patient information into the EMR with changes occurs
  • Runs active census’s weekly-schedules discharges with field staff and appropriate tasks to ensure accurate active census’s is up to date
  • Responsible for processing discharges-updating patients schedule to reflect discharge date and notifying medical records so they can break down patient’s active chart
  • Responsible for scheduling per physician order and ensuring re-authorization department has updated order in order to obtain authorization from physician office
  • Prints out all therapy evaluations from EMR-fills out fax cover sheet with recommendation and faxes to MD office ensuring effective communication with between physician’s office and home health agency
  • Participates with on-call rotation for home health approximately once per month
  • Performs other job tasks / duties as assigned by manager
