Job Description

The Canyon at Santa Monica is a center for recovery and healing. In addition to traditional recovery services, The Canyon provides a supportive environment where patients can drop in to work on life skills and seek support wherever they are in their recovery. Our unique treatment approach engages the whole person in the healing process and encourages loved ones to be involved in treatment in order to build a solid foundation for lasting recovery.

FRN is currently seeking an Intake Coordinator, to track patients through the admissions process, including treatment planning, aftercare planning, and discharge. This role serves as the primary interface with the referral source, family members, and the clinical team. The ideal candidate will be able to inspire our clients’ confidence in both our staff and the FRN Model of Treatment, by exhibiting high levels of both knowledge and compassion. Patient-centered care is not only what we do; it is at the core of who we are.

Job Requirements

Essential Duties and Responsibilities
• Assesses perspective patients for treatment, identifying psychosocial, mental health, and medical issues
• Provides directions or arranges appropriate transportation services as needed to transport patients
• Responsible for introducing new patients to Therapeutic Community
• Explains the evaluation and admission process to the potential patient/referral source and informs patient of necessary items to bring for treatment
• Documents all intake information into the TIER program, including demographics and all appropriate assessments
• Maintains daily census and provides assessment for appropriate intakes
• Ensures intake paperwork is complete and accurate, by creating intake files and properly filing intake-related documents
• Acts as role model by following program and treatment norms, including demonstrating honesty, a willingness to listen to patients’ issues, and taking responsibility for own actions
• Works collaboratively with the Call Center staff, Utilization Review staff, Nursing Staff, and Residential Counselors
• Maintains professional relations and assures information is communicated effectively with staff, as well as to patients, families, and referral sources
• Attends staff meetings, as indicated, and treatment team meetings, when necessary, to monitor results
• Administers breathalyzer upon intake and accurately documents the results
• Responsible for the accurate collection of co-pays, deductibles, and all other out of pocket cash collections (i.e. full self-pay payment plans), at the time of admission
• Acts as backup to Utilization Review Pre-Certifications

Knowledge, Skills, and Abilities
• Ability to inspire client confidence in the FRN Staff and the FRN Model of Treatment
• Ability to consistently demonstrate, through conduct and communications, the stability required to effectively assist patients and their family members through the FRN Model of Treatment
• Excellent verbal and written communication skills required
• Must have high level of interpersonal skills, as this position continually requires demonstrated poise, professionalism, tact, and diplomacy
• Exercise good judgment, apply common sense, and successfully handle ambiguity
• Understanding of diverse cultures and gender specific issues and the ability to incorporate needs of gender and culturally diverse groups into the organization
• Excellent organizational skills with the ability to prioritize workload and meet deadlines
• Must have strong knowledge of a variety of computer software applications in word processing, spreadsheets, and presentations (MS Word, Excel, and PowerPoint). Proficiency in MS Access (or other database program) is preferred
• Understands need for and maintains appropriate confidentiality at all times when interacting with patients, residents, families, visitors, referral sources, and all other contacts
• Exhibits excellent customer relations skills, as evidenced by supportive and constructive communication with all contacts including coworkers, patients, residents, visitors, families, and referral sources
• Knowledge of addiction, addictive process, and recovery
• Knowledge of group process, group facilitation, and conflict resolution
• Supports and backs up Utilization Review Specialist

Education / Experience
• Bachelor’s degree in psychology, social work, or related field required; Mental Health Service Provider preferred
• Minimum 1 year experience working in a psychiatric/substance abuse facility required
