Patients can self-reflect and find strength for life change amid the beauty of the expansive sky and the natural tranquility that characterizes our location.  FRN is currently seeking an Intake Coordinator to assist Black Bear Lodge with providing a level of therapeutic care that is admired throughout the mental health industry.  The ideal candidate will have the ability to Initiate and complete the intake process; completing all required paperwork, assessments and communication with intake department, Black Bear Campus staff and the Admissions Center.  Patient-centered care is not just what we do; it is at the core of who we are. Job Requirements

Position Summary

Initiates and completes the intake process completing all required paperwork, assessments and communication with intake department, Black Bear Campus and Admissions Center.   

Essential Duties and Responsibilities

May assess perspective patients for treatment, identifying psychosocial, mental health and medical issues to determine if patient meets admission requirements for Black Bear Lodge.

Completes Utilization Review Pre-Certifications based on thorough assessment completed by qualified Intake staff member or psychiatric evaluation completed by staff provider.

Administers Breathalyzer and Vital Signs upon intake; document accurate results. Provide results to nursing if results are elevated.

Provides directions and helps facilitate appropriate transportation services as needed to transport patients. 

Explains the evaluation and admission process to the potential patient/referral source.

Documents all intake information into the TIER program including demographics and all appropriate assessments.

Ensures intake paperwork is complete and accurate.  Creates intake files.  Files intake-related documents. 

Acts as role model by following program and treatment norms, demonstrating honesty and a willingness to listen to patients’ issues and taking responsibility for own actions.

Works collaboratively with the Admission Center staff and Black Bear staff to ensure safety and continuity of care for all patients.

Communicates and collaborates with Nursing/RA/Medical departments to facilitate an efficient transition of Patient into the treatment milieu.

Maintains professional relations and assures information is communicated effectively with staff as well as to patients, families and referral sources.

Attends staff meetings as indicated and treatment team meetings when necessary to monitor results.

Performs all other duties as required or assigned.

Knowledge, Skills and Abilities

Ability to inspire client confidence in the FRN Staff and the FRN Model of Treatment.
Ability to consistently demonstrate through conduct and communications the stability required to effectively assist patients and their family members through the FRN Model of Treatment.

Excellent verbal and written communication skills required.

Must have high level of interpersonal skills. Position continually requires demonstrated poise, professionalism, tact and diplomacy.

Exercise good judgment, apply common sense and successfully handle ambiguity.

Understanding of diverse cultures and gender specific issues and the ability to incorporate needs of gender and culturally diverse groups into the organization.

Excellent organizational skills with the ability to prioritize workload and meet deadlines.

Must have strong knowledge of a variety of computer software applications in word processing, spreadsheets, and presentations (MS Word, Excel, and PowerPoint).Proficiency in MS Access (or other database program) is preferred.

Understands need for and maintains appropriate confidentiality at all times when interacting with patients, residents, families, visitors, referral sources and all other contacts.

Exhibits excellent customer relation skills as evidenced by supportive and constructive communication with all contacts including coworkers, patients, residents, visitors, families and referral sources.

Knowledge of addiction, addictive process and recovery.

Knowledge of group process, group facilitation, and conflict resolution.
