Job Description
Fremont is a 148 bed acute psychiatric hospital situated in the heart of the bay area. We are a mid-sized facility filled with diversity and the feel of a close knit community. Fremont Hospital strives to be an integral part of the community, continually expanding our efforts to serve the psychiatric needs of adolescents and adults. We are seeking an Intake Nurse (LVN) for our part-time (Per Diem) NOC shift (11 pm - 7:30 am). As an active member of the Assessment and Referral Team, the Intake Nurse performs the initial medical screening assessments for incoming patients and coordinates with the Medical Staff in directing the patient towards an appropriate level of care. The responsibilities of this position include, but are not limited to: - Screening patients for admission into hospital programs, utilizing pre-approved admissions criteria. - Provide assessments of incoming patient’s mental health/psychiatric condition for mentally ill and/or emotionally disturbed patients - Prepare written reports of assessment results and prepares recommendations for the level of care and treatment needs - Complete an initial assessment on all assigned patients within eight (8) hours of patient’s admission utilizing all relevant data from other collateral assessment as appropriate - Maintains knowledge education regarding 5150/5585 criteria - Maintaining an organized environment with a courteous professional demeanor - Highly organized and detail-oriented - Excellent verbal, written and interpersonal communication skills
Job Requirements
Education: A master's in social work, psychology or related mental health field, PhD and PsyD candidates in from an accredited institution is required
