Position Summary:
JOB SUMMARY:

The Intake Clinician provides telephonic and in person risk assessment and crisis intervention, initial contact with referral sources and potential patients, coordinates the referral and intake process, determines appropriate level of care, facilitates the admission process and maintains communication with referral sources, families, patients and unit staff. The Intake Clinician also assists the Physician by providing face to face clinical assessments, including the history of presenting illness, family dynamics and supporting psychosocial information.

JOB SPECIFICATIONS:

The specifications listed below are representative of the knowledge, skill, and ability to meet the minimum requirements for this position. Staff must demonstrate knowledge and skill in provision of services to various populations including children, adolescents, adults, older adults and dually diagnosed patients in an inpatient and partial hospital setting.
 
Requirements
Education: Associate degree, licensed RN in good standing 

Experience: Prior intake, assessment, admission and crisis intervention experience with a psychiatric mental health population, preferably in an inpatient setting.

Licensure/Certification: RN

Knowledge: Prefer knowledge of crisis intervention, triage and risk assessment, psychiatric diagnoses, chemical dependency, age specific growth and development, crisis and behavior management.

Supervision Received: Director of Intake

Supervision Exercised: None

Environmental Conditions: Work in general office environment and hospital units/programs. Exposure to psychiatric patients’, who may exhibit violent/aggressive behavior. Potential for exposure to communicable diseases, blood/body fluids and other hazardous waste. General hospital environment.

Physical Requirements: Ability to communicate verbally, telephonically and in writing. Physical agility to manage patients. Push, pull and lift up to 40 pounds. Intermittent sitting, standing, walking, bending and stooping. Drug and alcohol free as evidenced by negative results of post-offer, pre-employment urine drug screen.
