Located in the heart of the Embarcadero district, Foundations San Francisco offers comprehensive, integrated outpatient treatment in a convenient urban setting. Flexible daytime and evening programs address addiction and any co-occurring mental health issues simultaneously in a supportive atmosphere. Patients are treated with dignity and respect as the staff focuses on each individual’s unique needs.

FRN is currently seeking an Intake/Patient Care Coordinator, to track patients through the admissions process, including treatment planning, aftercare planning, and discharge. This role serves as the primary interface with the referral source, family members, and the clinical team. The ideal candidate will be able to inspire our clients’ confidence in both our staff and the FRN Model of Treatment, by exhibiting high levels of both knowledge and compassion. Job Requirements

Essential Duties and Responsibilities
• Assists in coordinating preliminary evaluations with the Admissions Center for potential patients and ensures orientation of new patients immediately upon admission
• Documents all intake information into the TIER program, ensuring all intake paperwork is complete and accurate
• Collaborates with other departments to ensure safety, intake, and business needs are met
• Monitors the intake process, to ensure exceptional customer service regarding pre-orientation, referent contacts (including FRN referrals), and family updates
• Meets with the client within 24 hours, and opens a preliminary treatment plan for aftercare services
• Develops an aftercare plan in conjunction with the clinical team and the client
• Provides a summary of actions regarding “administrative” and “ACA” discharges
• Ensures information is communicated and updated to internal and external contacts (i.e. families, referring sources) on client progress, if/as authorized by the client in services
• Documents contacts and discharge according to periodicity and content requirements
• Responds to crisis, as needed, and develops a crisis plan upon client admission
• If the Office Manager is not available, then becomes responsible for the accurate collection of patient co-pays, deductibles, and all other out-of-pocket expenses and cash collections
• If necessary, administers urinalysis screening upon intake and documents accurate results


Knowledge, Skills, and Abilities
• Ability to inspire client confidence in the FRN Staff and the FRN Model of Treatment
• Ability to consistently demonstrate, through conduct and communications, the stability required to effectively assist patients and their family members through the FRN Model of Treatment
• Excellent verbal and written communication skills required
• Must have high level of interpersonal skills, as this position continually requires demonstrated poise, professionalism, tact, and diplomacy
• Excellent organizational skills, with the ability to prioritize workload and meet deadlines
• Ability to demonstrate understanding of a variety of models and theories of addiction, MI, and related issues
• Knowledge of philosophies, practices, policies, and outcomes of models of treatment, recovery, relapse prevention, and continuing care for dually diagnosed populations
• Must have strong knowledge of a variety of computer software applications in word processing, spreadsheets, and presentations (MS Word, Excel, PowerPoint)
• Understands need for and maintains appropriate confidentiality at all times when interacting with patients, residents, families, visitors, referral sources, and all other contacts
• Understanding of diagnostic criteria for dual conditions and ability to conceptualize modalities and placement criteria within the continuum of care
• Exhibits excellent customer relations skills, as evidenced by supportive and constructive communication with all contacts, including co-workers, patients, residents, visitors, families, and referral sources


Education and Experience
• Master’s degree in counseling or related field or equivalent experience
• Professional licensure/license eligibility and/or addiction related certification preferred
• 3-5 years’ case management experience required (with references)
• 2 years’ experience working in a co-occurring treatment setting or equivalent
• Marketing experience preferred
