FRN is currently seeking an Integrated System of Care Coordinator, providing an integrated continuum of care between UHS/FRN Psychiatric and Substance Use Disorder (SUD) Treatment Programs through patient assessment, engagement, transfer systems, reciprocal referral management and on site SUD education.  The ideal candidate will have the ability to inspire our clients’ confidence in both our staff and the FRN Model of Treatment, by exhibiting a high level of both knowledge and compassion. Job Requirements

Position Summary

  • Provides an integrated continuum of care between UHS/FRN Psychiatric and Substance Use Disorder (SUD) Treatment Programs through patient assessment, engagement, transfer systems, reciprocal referral management, and on site SUD education.
  • Is an employee of FRN and works onsite at the UHS Psychiatric Behavioral Health Hospital
  • Completes HR Orientation and site training at: Call Center, UHS Behavioral health facility, and FRN SUD Program.
  • This position is located at Del Amo Hospital in Torrance, California.

 

Essential Duties and Responsibilities

  • Provides SUD marketing materials to patients identified with SUD and completes brief assessment of SUD history.
  • Assesses patient criteria and demographics, reviews medical data and investigates insurance benefits.
  • Independently and collaboratively engages identified patients in a discussion about SUD treatment as part of their ongoing continuum of care.
  • Utilizes advanced knowledge of the System of Care and Stages of Change and other criteria to assess patients for appropriateness of cross-referral to SUD.
  • Coordinates with Admission Center to complete final clinical screening and verification of benefits
  • Uses MI to gain patient’s commitment to SUD treatment.
  • Upon request, participates in Clinical Pass Downs and Meetings at hospital to coordinate SUD aftercare treatment for patients identified.
  • Coordinates admission approvals, transfers and admission to SUD Program, including arranging transportation.
  • Develops and provides SUD psycho-educational groups to patients, IP and OP locations, as requested.
  • Responds to patient inquiries about SUD treatment.
  • Provides weekly check-ins with patients transferred from hospital to SUD.
  • Develops and provides individual SUD training and cross training for UHS hospital case managers and program staff.
  • Assesses patients for appropriateness of reciprocal referrals between UHS Psychiatric, and SUD facilities, including communication with family, referent, case manager, attending physician, and admission center.
  • Completes all required paper and electronic chart documentation ensuring accuracy and confidentiality of information.
  • Acts as role model by following program and treatment norms, demonstrating honesty and a willingness to listen to clients’ issues and taking responsibility for own actions.
  • May travels monthly to SUD Treatment Program for supervision.
  • Participates in monthly ISOC meetings
  • Performs all other duties as required or assigned.

 

Knowledge, Skills and Abilities

  • Ability to inspire patient confidence in the UHS Staff and the integrated system of care.
  • Ability to work autonomously and consistently demonstrate through conduct and communications the stability required to effectively assist patients, family members, referents, and co-workers through the integrated system of care.
  • Demonstrated proficiencies/skill sets in teaching and presentation of SUD psycho-educational lectures.
  • Excellent verbal and written communication skills required.
  • Must have high level of l interpersonal and customer service skills. Position continually requires demonstrated professionalism, tact and diplomacy.
  • Understanding of diverse cultures and gender specific issues and the ability to incorporate needs of gender and culturally diverse groups into the organization.
  • Excellent organizational skills with the ability to prioritize workload and meet deadlines.
  • Must have strong knowledge of a variety of computer software applications in word processing, spreadsheets, and presentations (MS Word, Excel, PowerPoint).Proficiency in MS Access (or other database program) is preferred.
  • Understands need for and maintains appropriate confidentiality at all times when interacting with clients, residents, families, visitors, referral sources and all other contacts.
  • Knowledge of psychiatric disorders and assessment for co-occurring disorders.
  • Knowledge of addiction, addictive process and recovery.
  • Knowledge of group process, group facilitation, and conflict resolution.
  • A current, valid driver’s license is required.

 

Education and Experience

  • Bachelor’s degree preferred.
  • Minimum of 2 Years of experience working in a psychiatric center, SUD program, or admission/intake department required.
  • Addictions Counselor licensure or related certification preferred.
  • Continuing education will be required.

 

Work Environment and Physical Demands

  • Facility environment M-F 8:00-4:30, with occasional alternative hours to meet the needs of patients and facilities
  • Some travel may require overnight stay
  • Sits, stands, lifts and moves intermittently during work hours.
  • Must be able to lift, push, pull and/or move up to 20 pounds.
  • Position requires routine work on the computer requiring manual dexterity and visual acuity.
