Job Description
Job Responsibilities: Provides a comprehensive range of Case Management and social work services to a variety of patients in an inpatient or partial hospital setting. Acts as the primary contact with external providers and participates in each assigned patient?s treatment planning. Conducts initial psychosocial assessments. Ensuring assessments, treatment plans and goals reflect integration and analysis of history and clinical observations. Actively identifies obstacles to achieve a positive outcome, initiates and implements treatment plans, working with treatment teams, supervisors and managers to resolve the obstacles. Provides group and family process orientated therapies using various professional treatment modalities. Coordinates discharge planning with patients, families and outside agencies as necessary. Attends multidisciplinary team meetings, and provides input to team members regarding psychosocial problems, overall treatment objectives, and treatment and discharge planning for each patient. ? Psychosocial Assessment: Initial psychosocial assessments are accomplished by the third working day from admission. Assessments, treatment plans and goals reflect integration and analysis of history and clinical observations. Plans are realistic, specific and goal oriented. Obstacles to plans or desirable outcome are identified. Assessment is complete through contacts with patients, families, outpatient providers, etc. Initial assessment and plans include adequate evaluation of living situation support networks, stressors and discharge planning. Responds to all emergency codes utilizing training in behavior management and/or CPR as well as other required training(s). ? Case Management: Begins work with patient/family on first business day in order to assess patients? living situations and identify cases presenting disposition problems. Documents need for current level of care in progress notes, and actively seeks alternative levels of care as indicated by assessment. Actively identifies obstacles to achieving positive outcome, initiates and implements treatment plans, working with treatment teams, supervisors and managers to resolve the obstacles. Provides group and family process orientated therapies using various professional treatment modalities. Coordinates discharge planning with patients, families and outside agencies as necessary, through individual and family meetings, telephone contacts and interagency conferences. These discharge planning interactions are documented in progress notes. Reviews/discusses discharge planning activities at team meetings, and documents these on weekly treatment plan updates. Attends multidisciplinary team meetings, and provides input to team members regarding psychosocial problems, overall treatment objectives, and treatment and discharge planning for each patient as reflected in written documentation. Acts in accordance with all federal and state reporting requirements to report all suspected incidents of abuse using provided forms and protocols. Schedules, attends and documents court hearings as necessary.
Job Requirements
Education: Masters degree required. Masters of Social Work or other relevant Masters degree preferred. Must be license-eligible. Licensed Independent
