Job Description


The LPN/LVN performs under the direction of a Registered Nurse in providing nursing care to assigned patients, utilizing basic understanding of psychiatric and personality disorders.




1.Accurately collect data needed for the appropriate portion of the integrated assessment.

2.Take and record vital signs properly.

3.Observe for, and document changes in patients appearance, behavior and ability to work toward treatment plan goals.

4.Communicate changes in patient’s condition to the RN.

5.Report signs and symptoms of infection in patients to the RN.

6.Verify the accuracy of assessments and medications to include MAR’s and medication reconciliation.

Treatment Planning

1.Maintain awareness of patient’s individualized plan of care including goals and interventions.

2.Provide information to the RN that assists in the development and revision of the treatment plan.

3.Document the treatment plan, nursing interventions and the patient’s response to the intervention.


1.Teach communication, interpersonal and social skills as well as the basic principles of physical health to the patient and significant others, under the supervision of a Registered Nurse.

2.Promote learning experiences and provides educational materials to patients and their families.

3.Re-enforce importance of medication compliance, and monitor compliance continuously.

4.Evaluate and documents the effectiveness of all patient teaching.

  1. Provide opportunities for patients and families to question, discuss and explore their feelings about prescribed therapies/interventions/diagnosis.

Milieu Management

1.Provide structure and maintains a therapeutic environment in collaboration with the patient and other healthcare providers.

2.Monitor patients and their therapeutic environment for changes in acuity and intervene if needed in a timely fashion.

3.Assure patients are adequately oriented to the unit.

4.Monitor unit activities and encourages patient participation.

5.Act as a role model to promote good health and hygiene skills.

6.Report potential crisis situations to the RN.

7.Consistently utilize universal precautions and environmental safety guidelines.

8.Maintain a safe environment by use of room checks.

9.Assist in structured didactic groups with patients and families, and prescribes, delegates, and coordinates nursing care to a group of patients under the supervision of a Registered Nurse.


1.Participate in orientation of all new staff.

2.Participate in the quality assurance program.

3.Attend shift meetings and assume responsibility for assigned groups of patients.

4.Collaborate with other team members on a continuous basis during shift.

5.Act as the patient's advocate and assure that patient rights are upheld.

6.Comply with established measures for infection control, OSHA regulations, and risk management.

7.Utilize time constructively, in an organized manner to accomplish assigned responsibilities, delegates duties to other personnel as appropriate, and demonstrate good judgment ability.

8.Follow all facility, departmental and personnel policies and procedures.

9.Comply with the Employee Code of Ethics.


  1. Employ principles of communication, interviewing techniques, problem-solving, and crisis intervention when performing psychotherapeutic interventions.
  2. Intervention is based on knowledge of psychiatric and medical nursing procedures, as well as the plan of care.
  3. Safely and accurately administer medications as prescribed.
  4. Transcribe and carry out physician's/licensed practitioner's treatment orders accurately.
  5. Anticipate and utilize appropriate intervention in psychiatric and medical emergencies.
  6. Organize and provide care to assigned patients.
  7. Perform general nursing procedures, treatments and report significant findings to the Registered Nurse.
  8. Meet with assigned patients daily and provide appropriate guidance.
  9. Utilize patient interaction, patient group activities, and psychiatric interventions when providing care.
  10. Organize and provide care based on patient needs and scheduled activities of the patient.


  1. Assure patients are adequately oriented to the unit and program.
  2. Monitor program activity and encourage patient participation.
  3. Coordinate nursing care to patients, during episode of care.
  4. Provide structure and maintain a therapeutic environment in collaboration with the patient and other healthcare providers.
  5. Consistently utilize universal precautions and environmental safety guidelines.
  6. Conduct structured didactic groups with patients and families as needed.


  1. Provide education to patients and families based on their identified needs and limitations, such as, medical problems, communication, interpersonal and social skills, as well as basic principles of physical health status.
  2. Provides and documents discharge instructions including food and drug interactions, medication regimen, medication reconciliation, diets and activities as appropriate.
  3. Evaluate and document the effectiveness of all patient teaching.
  4. Provide opportunities for patients and families to question, discuss and explore their feelings about prescribed therapies/interventions/diagnosis and medications.


  1. Participates in process improvement initiatives.
  2. Collaborate with other team members, units, and departments.
  3. Act as the patient's advocate and assures that standards of care, patient rights, and HIPAA are upheld.
  4. Comply with established measures for infection control, OSHA regulations, regulatory compliance, The Joint Commission, and risk management.   
  5. Utilize time management principles to accomplish assigned duties responsibilities.
  6. Demonstrates professionalism in communication, cooperation and provides assistance to patients, physicians/licensed practitioners, co-workers and visitors in a prompt and courteous manner. Will participate in service recovery initiatives and collaborate with grievance and complaint process.
  7. Follow facility, departmental and personnel policies and procedures
  8. Comply with the employee Code of Ethics/Code of Conduct.
  9. Will be available for emergency callback tree should an internal/external disaster occurs.
  10. Attends mandatory educational programs, minimum 75% of unit meetings, annual in-services/training and 100% of Health Stream assignments.
  11. Demonstrates the ability to recognize high-risk behaviors and documents supporting observations in the patient’s medical record.
  12. Ability to demonstrate critical thinking, recognize medical emergencies and act within the scope of practice.
  13. Will accept assignments and float to other units based on hospital needs.
  14. Performs other duties and responsibilities as assigned by supervisor.
  15. Maintains knowledge of assigned units rules, protocols and handbook guidelines.
  16. Relates to patients developmental stages as appropriate and or age appropriate activities.
  17. Utilize seclusion and restraint procedures with an understanding of the need for modification in regard to smaller body sizes and history trauma.
  18. Supports seclusion and restraint hospital policy and philosophy.
  19. Adhere to facility standards concerning conduct, dress, attendance, and punctuality.
  20. Maintain confidentiality of facility employees and patient information

Job Requirements


Education:    Graduate from an accredited professional vocational school of nursing or vocational training as a Psychiatric Technician, preferably with education in Child and Adolescent Development.

Experience: Minimum one (1) year as an LPN/LVN nursing preferred,

Licensure: Licensed to practice vocational/practical nursing in accordance with Texas State Board of Nursing.

Additional Requirements:  Must pass a medication administration exam before administering medications.  Must have current CPR certification upon hire.  Successful completion of Handle With Care to include physical hold and verbal de-escalation during orientation.
