Job Description




Resolves ACO quality and operational issues such determining performance, quality, and transition of care. Ensures all clinical operations comply with ACO guidelines and analyze data to improve the quality of care delivered. Maintains educational information databases for physicians and contracting party.  Reports to Director of Operations.



Performs pre-visits for review of health plan’s patient medical record to determine gaps in quality measures.
Conducts patient chart review in office if deemed necessary.
Prepares visit checklist to ensure accuracy of quality measures due to be completed on time.
Assists in a clinic service and collects relevant patient surveys and if applicable, reviews of patient medical record for past visits in calendar year.
Leaves a copy of records indicating services and results provided by ACO outcome guidelines.
Provides PVD, Neuropathy and COPD screenings.
Schedules follow up appointment with assigned PCP to ensure all results from the ACO guidelines are reviewed with the patient.
Conducts patient home visits as needed
Travels between facilities as requested
Other duties as assigned by management

Job Requirements




Familiar with standard concepts, practices, and procedures within a particular field. Relies on experience and judgment to plan and accomplish goals.

Performs a variety of tasks. A certain degree of creativity and latitude is required.

Knowledge of clinical terminology, clinical procedures and processes

Strong understanding and practice of HIPAA privacy law.

Required effective interpersonal skills and excellent written, oral communication skills required.

Understanding of functions and work flow of Clinic departments.

Excellent planning and organizational skills.

Demonstration of strong analytical skills and problem-solving abilities.

Strong teamwork skills and the ability to effectively communicate with all levels of staff.

Results oriented, independently goal directed, able to multi-task and meet established time frames.

Excellent customer service skills and must be able to work independently.



Graduate from an accredited LVN Nursing Program with a current LVN Texas License
Minimum of two years’ experience in a healthcare or clinical setting
