Position Summary: Primary function is to administer skilled nursing care, according to the physicians Plan of Care, under the supervision of a registered nurse, for clients in their place of residence, coordinate care with the interdisciplinary team, patient/family and referring agency.   

Job Requirements

Graduate of an accredited school of vocational nursing.

One year experience as a Licensed Vocational Nurse in a clinical care setting required.

Current Texas State license as a Licensed Vocational Nurse, current drivers license, auto liability and CPR required.

Secure in decision making and working independently.  Must be able communicate effectively in oral and written forms.  Knowledge of Medicare and Medicaid guidelines/requirements for home health.  Reliable transportation.  Valid and current auto liability insurance.
