Job Description

:

GENERAL PURPOSE OF JOB

The Licensed Practical Nurse renders practical nursing care to clients within the treatment programs.  Practices ongoing client observation of physical and behavioral symptoms and contributes to assessment and evaluation, and communicates pertinent data to the Registered Nurse and other service areas as needed.  Administers medications appropriately and documents all data accurately according to facility policy.  Participates in addressing individual treatment plans for all clients and implements plan according to facility policy. 

 

ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties may be assigned.

 

____ 1. Takes and records patients' vital signs. Provides appropriate evidence-based nursing care as indicated.

 

____ 2. Observes residents and reports adverse reactions to medication or treatment to medical personnel in charge.

 

____ 3. Administers specified medication, orally or by subcutaneous or intramuscular injection, and notes time and amount on patients' charts.

 

____ 4. Collects samples, such as urine from residents for testing.

 

____ 5. Transcribes physicians' orders from charts. Transcribes medication administration records.

 

____ 6. Communicates needs for medications from pharmacists and medical supplies from nursing supervisor.

 

____ 8. Conducts sick call or treatment screening.

 

____ 9. Documents all treatment by individual resident charting.

 

____ 10. Assists physicians and psychiatrists with visits to facilities.

 

____ 11. Schedules outside physician appointments for residents through transportation department.

 

____ 12. Administers first aid to staff and residents as required.

 

____ 13. Performs required vision and hearing tests.

 

____ 14. Performs new residents physicals, including, TB testing, required inoculations, and compiles medical histories.

 

____ 15. Counsels and educates residents on medical treatments and medications.

QUALIFICATIONS :

    1. EDUCATION and/or EXPERIENCE

        LPN license and certificate from college or technical school; with a minimum of three to six months related experience preferred.

 

    2. CERTIFICATES, LICENSES, REGISTRATIONS

        Valid Utah State LPN Nursing License  

 

     3. LANGUAGE SKILLS

        Ability to read and interpret documents such as safety rules, operating maintenance instructions and policy/procedure manuals.  Ability to respond to common inquiries or complaints from customers,     

        regulatory agencies, or members of the business community.  Ability to effectively present

        information to management, public groups, and/or boards of directors.

 

    4.  MATHEMATICAL SKILLS

         Ability to add, subtracts, multiply and divide in all units of measure, using whole numbers, common fractions, and decimals.  Ability to compute rate, ratio, and percent and to draw and interpret graphs.

 

     5. REASONING ABILITY

         Ability to solve practical problems and deal with a variety of concrete variables in situations where

         only limited standardization exists. Ability to interpret a variety of instructions furnished in written,

         oral, diagram, or schedule form.

 

    6. PHYSICAL DEMANDS 

        While performing the duties of this job, the employee is regularly required to walk, reach with hands

        and arms, talk or hear, and taste or smell.  The employee frequently is required to stand and use      

        hands to finger, handle, or feel.  The employee is occasionally required to sit; climb or balance; and

        stoop, kneel, crouch, or crawl.  The employee must occasionally lift and/or move up to 25 pounds. 

       Specific vision abilities required by this job include color vision, peripheral vision, and ability to

       adjust focus.

 

    7. PHYSICAL RESTRAINT 

         This position may require use of facility-approved restraint procedures to protect residents and/or staff.

 

     8. WORK ENVIRONMENT

         While performing the duties of this job, the employee is frequently exposed to outside weather conditions.  The employee is occasionally exposed to blood-borne pathogens and infectious diseases.  The noise level in the work environment is usually moderate.  Minimal local travel is required as deemed by supervisor. While performing the duties of this job, the employee may be exposed to clients who may ask inappropriate personal questions, display socially unacceptable personal behaviors, use profanity and sexually explicit phrases, make insulting remarks or threats regarding appearance, age, sex, or race, exhibit defiance, dishonesty, and assaultive or self-destructive behaviors.
