As a subsidiary of UHS, Windsor Laurelwood Center in Willoughby, OH is seeking a dynamic and talented full time Mental Health PHP Counselor.

At the Windsor-Laurelwood Center for Behavioral Medicine, we work together to provide the skill development, medication and therapies that individuals need to rebuild their lives.

 

Key Responsibilities include:

The PHP Therapist has primary responsibility for the day to day operation of the Partial Hospitalization Program.  The therapist coordinates the patient’s care through collaborative efforts with other team members, and is responsible for developing and monitoring the patient’s individual treatment plan.   He/ she must demonstrate the knowledge and skills necessary to implement the patient’s treatment plan, utilizing a variety of treatment modalities. The PHP therapist is responsible for communication with referral sources, outside community providers, and family members as needed, assuring adequate discharge planning and communication to the provider for the next level of care.

Job Requirements

::

Experience/Education:  Master’s Degree is required. A minimum of two (2) years of experience in mental health or substance abuse treatment is preferred.

Licensure:  Current unrestricted licensed in the State of Ohio to provide mental health and/or drug and alcohol services as appropriate to specific program assignment. Specific Chemical Dependency training and/or credentialing is required if employee is assigned to Chemical Dependency treatment program. 

Additional RequirementsTrained in non-violent physical crisis intervention and CPR prior to accepting independent assignment.
