Poplar Springs Hospital, a 183 bed acute care and residential facility, is located in Petersburg, Virginia, 20 minutes south of Richmond and 50 minutes north of North Carolina. Situated on 25 acres, our serene, nature-like setting offers our patients a sense of calmness and peace. For more than 30 years Poplar Springs Hospital has been guided by its mission of helping those who need the full continuum of behavioral services. Our facility offers behavioral and psychological treatment for adults and adolescents through our acute programs, adolescent residential programs, sexually abusive youth services, as well as programs for active duty military.

The candidate selected will be responsible and accountable for:

The primary role of the Care Coordinator is to help coordinate admissions, during stay, and after-care needs of each service member, veteran, or dependent based on recommendations of the clinicians, base commands, and patient’s goals. This is a person-centered process which is driven by an individualized treatment plan that functions to maintain the individual’s well-being and prioritize treatment goals and objectives. The Care Coordinator will familiarize him/herself with community and military resources and draw upon them through linkages and referrals to assist patients in accessing the network of resources that will help maintain their community and/or military status and promote further growth to maximize functional independence.

The Care Coordinator completes all processes, procedures, and case management activities directly related to admission, treatment and discharge from the Military program. The Care Coordinator will work collaboratively with the Director of Military Programs to ensure timely communication with military bases and referral sources.  They will ensure all documents are sent to the appropriate team or agency and all follow-up appointments are annotated on the service member’s discharge instructions. The Care Coordinator will back-up the Intake Coordinator to ensure referrals are responded to and patients are admitted on a timely basis. The Care Coordinator also works the clinicians, nurses, psychiatrists, service members, military and community agencies as needed, with the goal of linking patients to appropriate continuing care resources such as behavioral health, ASAP, Social Work or CAFAC. The Care Coordinator acts as a member of a multi-disciplinary treatment team, and serves as an advocate for the patient.

The Care Coordinator will complete chart audits to ensure proper documentation and compliance with local, state and federal regulations. Employee shall function in compliance with all organizational policies and observe ethical and confidentiality standards at all times. This position will work closely with members of the therapeutic treatment team, external agencies, the military, clients, families, and members of other departments. The Care Coordinator will also be trained as the backup admission coordinator to ensure continuity of care in the absence of full time staff.

Job Requirements
  • Minimum of a Bachelor’s Degree in Human Services or related field.
  • Minimum of two years of experience in the healthcare setting
  • Extensive knowledge of military culture
