Peachford Hospital is seeking a dynamic and talented Mobile Assessment Counselor.

Located in the beautiful Dunwoody neighborhood of Atlanta, Peachford features individual units for children, adolescents, adults and seniors, serving residents of Georgia and beyond. We offer inpatient acute care, as well as partial hospitalization and intensive outpatient programs. (www.peachford.com)

The Mobile Assessment Counselor will provide mobile face-to-face assessments, recommend appropriate level of care and provide crisis intervention services.

Key Responsibilities include:

  • Participate in On-Call monthly schedule.

  • Conduct an initial face-to-face assessment on site, arriving at the site no later than 60 minutes after receipt of the call.

  • Formulate level of care recommendation and immediate patient/client disposition.

  • Make arrangements for immediate disposition regarding transfers/discharges.

  • Provide primary contact with patient and family until transfer/discharge occurs.

  • Serve as a provider of crisis intervention by meeting with individuals and/or family to address the crisis and identify triggers for crisis behavior.

  • Serve as a liaison between mobile assessment/crisis intervention and Assessment and Referral Services.

  • Stay abreast of hospital programs and services.

  • Serve as a back-up team for other sites requiring assessment and referral services.

  • Adhere to all safety policies and safe work practice.

  • Adhere to all hospital policies and procedures.

  • Perform other duties as assigned.

  • Support facility internal and external customer service standards.

  • Follow Peachford Hospital Time and Attendance policy.

Job Requirements

Individuals must hold a Master’s Degree in a related field. Psychiatric assessment experience with all age populations preferred. LAPC, LMSW, LPC, or LCSW preferred.
