Millwood Hospital  is seeking a dynamic and talented Mobile Assessment Team Evaluator

Position Summary: The Mobile Assessment Team Evaluator (“Assessor”) provides mental health assessments for contracted emergency rooms and/or hospitals, recommends the appropriate level of treatment based on the Assessor’s assessment of risk factors, and triages patients to appropriate services.

Key Responsibilities include:

Patient Assessment       

 1.     Participate in screening patients for admission into hospital programs, utilizing pre-approved admissions criteria.

2.      Provide assessments of incoming patient’s mental health/psychiatric condition for mentally ill, emotionally disturbed, through various methods.

3.      Prepare written reports of assessment results and prepares recommendations for the level of care and treatment needs.

Treatment Planning

 1.     Evaluate patient needs and advocate the lowest level of care consistent with the defined patient needs.

 2.     Interface with the facility staff who will provide the appropriate level of care, to ensure a smooth transition for the patient.

 3.     Promote an effective use of resources for patients, customers and the facility with sensitivity to cost of health care.

 4.     Observe patient to detect behavioral patterns and report observations to medical staff.

 5.     Assist with completing initial admissions forms for patients as required.

 6.     Intervene to restrain violent or potentially violent or suicidal patients by verbal or physical means as required.

 7.     Contact patient’s relatives by phone to arrange family conferences as required in a timely fashion.

 8.     Begin patient/family education at time of admission.

 9.     Provide information about the patient to the program staff who will be receiving the patient.

10.    Ensure proper tracking of all reviews.


Job Requirements


Education:  High School Degree or equivalent preferred; or Certified Nursing Assistant preferred; Degree from an accredited college or university in social work, recreational therapy or appropriate therapeutic discipline.

Experience: Two (2) years of related experience, with a knowledge of psychiatric patient care techniques with understanding of mental illness, preferred.

Licensure:   Must maintain a valid driver's license in the applicable state.

Additional Requirements:  Successful completion Management Aggressive Behavior Training (CPI) before on the unit orientation and prior to assisting with restraining procedures . May be required to work overtime and flexible hours.
