NICU Social Worker - Telecommute (748961)

Social workers are responsible for providing social work services to patients and families.  They function as a member of the departmental team which includes educating the patient, family and members of the healthcare team regarding benefits, community resources, referrals for counseling and other pertinent information.  Social workers are also responsible for triaging referrals and collaborating on cases with other members of the healthcare delivery team.  In addition, social workers must be able to assist the patient in a sensitive and supportive manner, while acting as an advocate on behalf of the patient.

Primary Responsibilities:

  • Communicate directly with families of babies currently admitted or recently discharges from the NICU, physicians and community / governmental agencies
  • Collaborate with NICU nurses regarding barriers or challenges families are confronting and assist with solution
  • Through direct telephonic assessment, assist family members to continue to function with their babies illness impacts their physical health, mental status and social role
  • Assist families in ing for Social Security Disability Benefits
  • Asses for social, economic and emotional factor
  • Assist members and families in discharge planning where necessary
  • Develop working knowledge of resources readily available to interdisciplinary team and families
  • Promote effective utilization and monitoring of community resources and assist in optimization of available benefits including referrals to outside vendor
  • Providing education on community, Federal and State resources

This is a Monday - Friday position working traditional business hours as a telecommuter.

Required Qualifications:

  • Masters in Social Work (MSW) with active and unrestricted social work license in the state of residence
  • 2+ years of clinical experience in a neonatal or NICU setting
  • Very strong oral and written communication skills for a variety of audiences (families of babies in NICU, providers, community resource)
  • Knowledge of pertinent websites, community resources, and other ways to provide assistance to consumers
  • Proficient Computer skills in Microsoft Office such as Word, Outlook and Internet

Preferred Qualifications:

  • Recent experience in discharge planning
  • Certified Case Manager (CCM)
  • Experience in Managed Care / Insurance (MCO)
  • Knowledge and experience with Medicaid programs and regulations
