We’re searching for an experienced Social Worker to fill an exciting role in our Neonatal Resources Services program, with emphasis in serving Medicaid population. This position provides telephonic support to families of babies receiving services through the neonatal intensive care until (NICU).  You will have the ability to make a significant and positive difference in the lives of the families we serve.


***This is a Monday – Friday position working traditional business hours as a telecommuter ***

Primary Responsibilities:

  • Communicate directly with families of babies currently admitted or recently discharges from the NICU, physicians and community / governmental agencies
  • Collaborate with NICU nurses and / or NICU SW / Discharge planners regarding barriers or challenges families are confronting and assist with solution to prevent delay in infant’s discharge
  • Through direct telephonic assessment, assist family members to continue to function with their babies illness impacts their physical health, mental status and social role
  • Assist families in ing for Social Security Disability Benefits and/or Medicaid Waiver programs.
  • Assess for social, economic and emotional factor
  • Assist members and families in long - term care planning where necessary
  • Develop working knowledge of resources readily available to interdisciplinary team and families
  • Promote effective utilization and monitoring of community resources and assist in optimization of available benefits including referrals to outside vendor
  • Providing education on community, Federal and State resources
  • Assess for post partum depression by completing a Patient Health Questionnaire (PHQ2 / PHQ9)

Required Qualifications:

  • Masters in Social Work (MSW) with active and unrestricted social work license in the state of residence
  • Very strong oral and written communication skills for a variety of audiences (families of babies in NICU, providers, community resource
  • 2+ years recent experience in discharge planning
  • 3+ years of clinical experience in the neonatal / NICU setting
  • Knowledgeable of pertinent websites, community resources, and other ways to provide assistance to consumers
  • Proficient Computer skills in Microsoft Office such as Word, Outlook and Internet

Preferred Qualifications:

  • Certified Case Manager (CCM)
  • Experience in Managed Care / Insurance (MCO)
  • Knowledge and experience of Medicaid programs and regulations
