This is a Telecommute / Work from home position covering the greater Northeast Wyoming area.  This position is both Telephonic and Field based with potential to be in the field up to 50% of the time going into homes and facilities to meet with members. The estimated territory for this position will be within 100 miles of your home. Candidates must be licensed in and reside in the state of Wyoming.  When not in the field this is a work from home position.  Core hours are Monday - Friday, 8am - 5pm.


As one of our newest Field based and Telephonic Case Managers, you'll wear many hats, and work in a variety of environments. Sometimes, you'll interact with members leaving the hospital  -  possibly with new medications or diagnoses. Or perhaps you'll perform home visits (as needed), assisting members with safe, effective transitions from care environments to where they live. You may also act as an intermediary between providers and members  -  serving in numerous roles, such as educator, evaluator, service coordinator, community resource researcher and more. The result? Fewer hospitalizations, ER visits and costly service gaps; and a less stressed, more effective health care system for us all.


Primary Responsibilities:

  • As needed, visit Medicaid members in their homes and / or other settings, including community centers, hospitals or providers' offices
  • Provide a complete continuum of quality care through close communication with members via in - person or over the phone interaction
  • Support members with condition education, medication reviews and connections to resources such as Home Health Aides or Meals on Wheels
  • Assist members with the transition from a care facility back to their home
  • Interaction with primary care providers and Patient Centered Medical Homes (PCMH) in your assigned territory to collaborate on individual clients and their care.

This position will require active and unrestricted Nursing licensure in multiple US States.  Selected candidate must be willing and able to obtain and maintain multiple state licensure. (Application fees and filing costs paid for by UHG)   


Required Qualifications:

  • Current, unrestricted RN license in the State of Wyoming
  • Multiple state licensure (in addition to Compact License if applicable) or ability to obtain multiple state nursing licenses
  • Ability to travel to member homes or other locations within 100 miles of residence as needed
  • 3+ years of RN experience
  • Ability to create, save and edit documents utilizing Microsoft Word
  • Must be able to navigate a Windows based environment and conduct internet searches
  • Must have access to high speed internet for home office set up

Preferred Qualifications:

  • Case Management, Utilization Management or Prior Authorization experience
  • Certification in Case Management (CCM)
  • BSN or greater
  • Home care or field based case management experience
  • Medicaid knowledge or experience
  • Experience working with the needs of vulnerable or fragile populations who have chronic or complex bio - psychosocial needs
  • Public health experience
  • Home and Community based or Long Term Care services delivery
