Position Description

UnitedHealthcare (TeamMD) is recruiting for an experienced Nurse Practitioner (NP) to work with myConnections to provide integrated health and social services to individuals who are homeless or at risk of homelessness. The greatest barriers to better health for many people enrolled in Medicaid are not clinical issues, but rather social or financial barriers, such as lacking access to safe and affordable housing or having no way to travel to a job or needed services. myConnections integrates, organizes and distributes support services for consumers and provides people with greater access to safe housing, transportation, education and job training, which are essential to good health.


The successful NP candidate will be embedded within a transitional housing complex and will work closely with an interdisciplinary team to assess individual needs, help individuals transition safely from hospital or homeless shelter, provide post-acute care, and coordinate referrals and services with local primary care and community based organizations.


This NP must have demonstrated compassion and mission driven attributes with at least two years of post-licensure experience, at least one year of experience working with individuals living with mental illness and/or substance addictions, and awareness of working in a trauma informed care environment. Strongly prefer experience working with homeless individuals.  Work environment is primarily in home (in transitional low income housing), with some field work, including homeless shelters, CBOs, etc. The ideal candidate will have experience in full scope family medicine with a special interest and familiarity in meeting the medical needs of patients with complex behavioral, social and / or functional needs.


Primary Responsibilities:

  • Provide temporary stabilization services for post-acute and transitioning patients
  • Participate in collaborative multidisciplinary team meetings to optimize clinical integration, efficiency, and effectiveness of service delivery
  • Manage medical emergencies, triage phone calls, prescribe medications, and make referrals for ongoing services 
  • Provide consultation to other providers either in person or by telephone
  • Work closely with psychiatrists and mental health clinicians to help bridge the gap between mental and physical health
  • Consult with clinic physicians, specialists, or other health care professionals as appropriate
  • Assess patient needs for specialty referrals, diagnostic and in-patient care and make appropriate referrals for service
  • Completely and accurately document and code for services provided and patient conditions
  • Facilitate clarification of patient goals of care to support development of a comprehensive care plan
  • Provide education and information on health conditions, illness trajectory, and treatment options to patients, families, caregivers, and providers
  • Provide input to organizational quality initiatives and staff education
  • Maintenance of credentials essential for practice, to include licensure, certification and CME
  • Provide patients and family members with counseling and education regarding health maintenance, disease prevention, diagnosis, treatment, and need for follow up as appropriate during each patient visit. Verify and document that the patient understands diagnosis, treatment and follow up recommendations
  • Demonstrate a commitment to the mission, core values and goals of UnitedHealthcare and its healthcare delivery including the ability to integrate values of justice, respect, compassion, excellence and stewardship into appropriate programs and services

 Required Qualifications:

  • Graduate of an accredited Master's Degree in Nursing (MSN) program
  • Board Certified through the American Academy of Nurse Practitioners (AANP) or the American Nurses Credentialing Center (ANCC)
  • Current unrestricted nursing license in good standing in Arizona
  • Ability to obtain DEA licensure / Prescriptive Authority post-hire in states where applicable
  • 2+ years in practice as a Nurse Practitioner
  • 1+ years experience working with individuals with mental illness and/or substance abuse
  • Ability to integrate values of justice, respect, compassion, excellence and stewardship into provision of services
  • Ability to work with diverse care teams in a variety of settings
  • Advanced knowledge of and experience with symptom management
  • A clinical background in adult, family or geriatric specialties
  • Experience working with patients in non-clinical settings
  • Computer literate and experience with Electronic Medical Records
  • Access to reliable transportation that will enable you to travel to client and/or patient sites within a designated area

Preferred Qualifications: 

  • Experience in management of chronic conditions
  • Experience working with individuals who are homeless and / or underserved
  • Home care or home visit experience
  • Effective communication skills with the geriatric or Medicare population
  • Spanish Bilingual
