Job Description
Fremont Hospital is a 148 bed acute psychiatric hospital situated in the heart of the bay area. We are the largest freestanding psychiatric hospital in the East Bay filled with diversity and the feel of a close knit community. We currently seek an experienced Clinical Case Manager to fill our open Part-Time and Per Diem positions. The Clinical Case Worker I & II perform clinical services functions by providing ongoing support and expertise through comprehensive assessment, planning, implementation and overall evaluation of the individual patient needs. The overall goal of the position is to enhance the quality of management and satisfaction, to promote continuity of care and cost effectiveness through the integrating and functions of case management, utilization review, and management of discharge planning. Has accountability for the care, coordination and discharge planning of all patients on assigned unit(s). The OPS Therapist is assigned a caseload of patients. The responsibilities of this position include, but are not limited to: assessment, discharge planning, reporting during treatment planning, maintaining patient attendance to prescribed treatment, following-up to help ensure the safety of all patients, and ensuring documentation compliance. The Therapist coordinates the initiation of a Master Treatment Plan and updates that are consistent with the findings of initial assessments, ongoing assessments, treatment team meetings and other documented observations. The Therapist attends daily utilization review/clinical meetings and ensures that the behavioral treatment provided to the patient is consistent with what is communicated to and understood by the Utilization Review Specialist, management and other team members and that this information is consistently documented in the chart across disciplines. The Therapist facilitates groups and 1:1 meetings using an evidence-based treatment consistent with research findings. The Therapist exercises sound judgment and clinical excellence, sustains therapeutic alliance and patient satisfaction. The Therapist will be cross-trained and assigned, as determined by department needs, in the Intake and Utilization Review functions.
