As a Case Manager RN, you'll wear many hats and work in a variety of environments. Sometimes, you'll interact with members leaving the hospital - possibly with new medications or diagnoses. Or perhaps you'll perform home visits, assisting members with safe, effective transitions from care environments to where they live. You may also act as an intermediary between providers and members - serving in numerous roles, such as educator, evaluator, service coordinator, community resource researcher and more. The result? Fewer hospitalizations, ER visits and costly service gaps; and a less stressed, more effective health care system for us all. Want more flexibility, want more autonomy? This is a field position visiting patients in the community.  We have a few openings; 2 that are 100% based in the field and one that is a combination of field and office work.  Office location likely to be Sunrise Marketplace in Las Vegas.

 

Primary Responsibilities:

  • Provide a complete continuum of quality care through close communication with patients via in-person or phone interaction
  • Support patients with condition education, medication reviews and connections to resources such as Home Health Aides or Meals on Wheels
  • Visit patients in their homes to discuss goals and available resources
  • Use a team approach to address the needs of patients and their families, including bereavement counseling, if indicated
  • Offer a support system to help the family help during patients’ illness and in their own bereavement
  • Provide relief from pain and distressing symptoms
  • Other responsibilities as assigned
