The Patient Care Coordinator - Outreach is responsible for initial outreach to health plan members to engage them in Optum’s clinical programs. 
Primary Responsibilities:
  • Outreach to all health plan members to explain clinical programs
  • Enrollment of new patients into systems
  • Demonstrates a high level of customer service behavior at all times
  • Participates in quality monitoring process
  • Processing referrals for durable medical equipment and referrals to specialist
  • Patient satisfaction calls
  • Expedites emergency calls as needed
  • Attends regular staff meetings as scheduled
  • Maintains confidential health information according to state and federal regulations including HIPAA
  • Responsible to assist with gathering and organizing of appropriate medical records (e.g. history and physical, lab work, test results)
  • Filing and faxing
  • Other duties as assigned by supervisor
Required Qualifications:
  • High School Diploma / GED or higher
  • Ability to crate, copy, edit, send, and save within Microsoft Word, Microsoft Excel, and Microsoft Outlook
  • Knowledge of Medical Terminology
  • Able to type a minimum of 25 WPM
  • 2+ years of previous experience in healthcare or customer service related experience
  • Telephonic customer service experience
  • Ability to work Monday - Friday from 8:30am – 5:00pm

Preferred Qualifications:

  • Experience working with the geriatric population 
  • Health insurance experience
  • Call center experience

Soft Skills:

  • Excellent written and verbal communication skills
