Job Description


The clinical pharmacists keep the patient and their wellbeing as the central focus of their professional activities.  The clinical pharmacists are responsible for providing drug therapy review/monitoring, prescribing, preparation, counseling, dispensing and drug control in accordance with departmental and hospital policies and procedures as well as all applicable laws and regulations.  These responsibilities require excellent communication skills with patients and their families, medical staff members and department/hospital staff.  The clinical pharmacist also takes an active, participating role in the development and implementation of new programs, while supporting and facilitating the programs in place.

  • Interprets and verifies medication orders (verbal and written) and transcribes into Cerner PharmNet computerized patient medication profiles accurately.  Maintains accurate, complete pharmacy patient medication profiles.
  • Demonstrates CE completion about developments in acute care pharmacy and health care that relate to job responsibilities and integrates them into their practice.
  • Consistently takes action on targeted medications for compliance with current TVH pharmacoeconomic programs.
  • Monitors drug therapy regimens and makes interventions for significant contraindications, drug-drug interactions, drug-food interactions, allergies, adverse reactions and appropriateness of drug, dosage form, regimen route, and therapeutic duplications.
  • Enforces the hospital drug formulary, minimizing non-formulary procurements, utilizing therapeutic substitution protocols and promoting rational drug therapy selection.
  • Supervises and directs pharmacy support personnel.  Verifies the daily activities of pharmacy technicians. Verifies the accuracy of all orders and IV admixtures prepared by the technician.
  • Provides consultation to practitioners for clinical drug use and drug information.  Suggests appropriate, cost-effective therapeutic alternative to medical staff as needed.
  • Dispenses and maintains a perpetual inventory for all controlled substances and maintain records as required by State and Federal law.
  • Collects data, conducts inspections and maintains logs and other documentation as assigned.
  • Participates in the development and presentation of orientation, education and training programs for the medical staff, nursing service, new employees and other hospital staff.
  • Responsible for providing quality pharmaceutical care to inpatients and outpatients.
  • This includes all aspects of therapeutic selection, implementation, monitoring, modification and dispensing of drug therapy - all with appropriate documentation and communication.
  • Responsible for documenting drug information questions, drug utilization evaluation activities and adverse drug reactions.
  • Assures that proper charging documentation is completed correctly for services provided.
Job Requirements


  • Maintains competency in all aspects of drug therapy including factors related to patients of all age groups including adolescents, adults and geriatrics.
  • Knowledgeable of and complies with laws, rules and regulations that govern the practice of pharmacy.
  • Pyxis experience with knowledge of Pyxis Reports (i.e., optimization, load/unload, nursing narcotic access, etc.)



  • Minimum of 1-year experience as a Pharmacist or Health System Residency completed
  • Preferred 1-3 years of Hospital experience.



  • Minimum Bachelor’s Degree in Pharmacy (B.S. Pharm.)
  • Preferred Doctor of Pharmacy Degree (Pharm. D.)
  • Preferred ASHP accredited residency



  • California Board of Pharmacy – Registered Pharmacist
  • Current BLS (AHA) within 30 days of hire
  • Current ACLS (AHA) within 30 days of hire



  • Staff Pharmacist provides comprehensive pharmaceutical care through integrated drug distribution, patient counseling and teaching.
  • Staff Pharmacists verify CPOE orders, fill and check orders for medications, monitor patient’s drug therapies and provide drug information.
  • Staff pharmacist supervises and directs supportive staff.
  • In the absence of the Pharmacy Director, a staff pharmacist may be required to assume the essential responsibilities and perform the duties of the Pharmacy Director.
